Job Description
Ebenezer Integrated Care & Rehab (EICR) is a dedicated healthcare establishment located in the heart of downtown St. Paul, focusing on providing exceptional care and support to seniors. As part of the Fairview Community Health and Wellness Hub, which opened in 2022, EICR is committed to delivering innovative and collaborative care services tailored to seniors with long-term, transitional, and dementia care needs. The Hub integrates various Fairview services with local organizations such as Minnesota Community Care and Sanneh Foundation to provide comprehensive access to healthcare, mental health services, enrichment programs for seniors, food access initiatives, and community gathering spaces. This approach harnesses a community-centered model that aims to improve the well-being of seniors while creating a supportive network that promotes health and wellness in a holistic sense.
The Adult Day Program Coordinator role at EICR is vital in ensuring the smooth and effective operation of the Adult Day Program. This full-time position is scheduled for 80 hours every two weeks, Monday through Friday from 7:45 am to 4:15 pm, with no weekend hours required, making it ideal for professionals seeking a balanced work-life schedule. The coordinator plays a key role in fostering socialization among participants and assists in planning and implementing a variety of engaging programs tailored to the needs of individual attendees and groups. Responsibilities include managing participant charts and case files in compliance with licensing regulations, supporting families and case managers, overseeing program assistant charting for accuracy and timely updates, and participating in the development of care plans. The coordinator is also responsible for organizing and conducting tours, assisting with enrollment paperwork, and performing quarterly and annual assessments as directed to maintain high standards of care.
In addition to administrative and coordination duties, the Adult Day Program Coordinator provides direct care to the program participants. They are involved in preparing, implementing, and cleaning up daily activities such as crafts, woodworking, games, and music, which are designed to encourage active participation and cognitive engagement. They also manage snack and meal preparation, ensuring nutritional needs are met and assisting participants with feeding when necessary. Personal care responsibilities include assisting with grooming, bathing, and toileting needs, promoting dignity and independence among participants. The role involves continuous communication with staff, participants, and families to monitor health status, provide counseling, and address concerns promptly. Reporting progress and any issues to the Director is a critical aspect of this leadership position, which also functions as the lead in the Director's absence.

Job Requirements
- CPR certification
- Basic first aide training
- Ability to provide direct care to seniors including personal hygiene and meal assistance
- Experience in case management or healthcare setting preferred
- Strong communication and organizational skills
- Ability to work full time Monday through Friday from 7:45 am to 4:15 pm
- Ability to assist with care plan development and program coordination
- Reliable and compassionate with attention to detail
Job Qualifications
- CPR certification
- Basic first aide training
- Associate of arts (A.A.) or Associate of science (A.S.) degree preferred
- Less than 1 year of relevant experience preferred
Job Duties
- Assist the director in the efficient operation of the program including conducting tours, meeting with families, assisting with enrollment paperwork, communicating with case managers and families, overseeing charting of program assistants, assisting with care plan set up, conducting quarterly and annual assessments, assisting with office duties such as attendance and billing, functioning as lead staff in the director's absence
- Responsible for individual case management including setting up participants' charts as per licensing requirements, ensuring forms are completed and updated, maintaining monthly charting and quarterly care plans, documenting monthly health education, fire drills, blood pressures and weights for each participant
- Provide direct care for adult day program participants including preparation, implementation and clean up of daily programs such as crafts, woodworking, games, and music
- Provide snacks and noon meal to participants including set up, cutting up food, feeding participants as necessary and clean up
- Assist staff in developing monthly calendar
- Responsible for daily menu planning and ordering of meals and snack supplies
- Perform routine personal care and grooming at the center including baths
- Assist identified participants with all toileting needs
- Promote and encourage individual care
- Provide individual and family counseling for participants as needed
- Communicate with staff regarding participants' needs and individual plans of care
- Observe, assess and intervene with participants' health concerns
- Report any problems, progress and observations to the director
