We are currently seeking a Certified Nursing Assistant (CNA) to join our PACE team. This is a full-time, in-person role based in Baton Rouge, designed for compassionate individuals with a passion for supporting elderly individuals in maintaining their independence and quality of life. As a CNA with PACE Baton Rouge, you will play a pivotal role in delivering hands-on personal care and daily support to participants both in the PACE Center and in their homes. Your responsibilities will encompass a wide range of participant care activities including assisting with daily living activities, monitoring medication schedules, providing companionship, and supporting therapeutic exercises under the supervision of licensed therapists. You will be instrumental in helping participants with tasks such as bathing, dressing, transferring, and vital sign monitoring, ensuring their comfort and safety at all times.

The position also demands a dedication to maintaining a clean and safe living environment by performing light housekeeping tasks, including cleaning, laundry, and sanitation according to the participants' care plans. Your role will involve collaborating closely with a multidisciplinary team to observe, report, and communicate any changes in participants' conditions, ensuring timely and appropriate interventions. This role requires proficiency in electronic health record documentation for accurate daily record-keeping and adherence to confidentiality and participant rights guidelines.

Furthermore, as a CNA on our PACE team, you will facilitate participant engagement in therapeutic and social activities, encouraging mental and physical stimulation through walks, reading, and accompanying them to medical or other appointments. You will also help with errands and grocery shopping, contributing to a comprehensive care approach that supports participants' independence and wellbeing.

This position requires a valid, unrestricted Louisiana driver’s license and reliable transportation to fulfill the home visit requirements effectively and safely. Mandatory participation in ongoing education and in-service programs is also emphasized to ensure up-to-date care standards in compliance with CMS regulations.

Job Requirements

  • One year of CNA or personal care experience working with frail or elderly population
  • High school diploma or equivalent

Job Duties

  • Perform participant care activities in both home and center settings
  • Record pertinent information related to participant care
  • Remind participants to take medications on schedule
  • Plan, prepare, and serve meals according to dietary requirements
  • Assist participants with therapy exercises under therapist direction
  • Support activities of daily living including transferring, bathing, dressing, catheter and skin care
  • Assist with ambulation with or without mechanical aids
  • Maintain a safe, clean, and healthy environment through light housekeeping tasks
  • Provide companionship and stimulation including reading and walks
  • Accompany participants to medical and other appointments
  • Perform grocery shopping and errands as per care plan
  • Observe and report changes in participants' condition
  • Follow participant rights and confidentiality guidelines
  • Prepare and submit accurate daily records
  • Participate in mandatory in-service education
  • Maintain cleanliness and safety in the Adult Day Center
  • Collaborate with the PACE interdisciplinary team
  • Actively participate in unit teams and committees
  • Attend all mandatory CMS-required in-services
