Job Requirements

  • High school diploma or equivalent
  • Minimum of one year experience working with a frail or elderly population preferred
  • 3-5 years administrative and clerical experience preferred
  • Current and unrestricted driver’s license preferred
  • Strong collaborative and interpersonal skills
  • Ability to manage multiple deliverables concurrently
  • Proficiency with Microsoft Office
  • Must demonstrate integrity and personal commitment
  • Ability to function in a matrix environment
  • Excellent verbal and written communication skills

Job Duties

  • Participates in initial admission screenings, identifying needs, and meeting with family members
  • Acts as a liaison and resident advocate by addressing any issues and concerns among residents, families, staff and outside resources
  • Coordinates individual and family meetings as needed or prescribed in the plan of care
  • Maintains appropriate documentation in the clinical and business records per ministry policy
  • Performs other duties as assigned
