The role of the Admission Assistant at Catholic Care Center is a vital part-time position focused on facilitating a smooth and supportive admission process for new residents. Working every Friday and Saturday from 9:00am to 5:00pm, this role demands someone who is patient, detail-oriented, and skilled in communication. The Admission Assistant is responsible for identifying the psycho-social needs of prospective residents during initial screenings, working closely with families and other caregivers to ensure all parties are well-informed and comfortable with the care plans.

This position acts as a crucial liaison between residents, their families, staff members, and external community resources, including agencies such as the Department of Aging, Social Security Administration, Medicaid, and others. Responsibilities include coordinating individual and family meetings, maintaining accurate documentation in clinical and business records, and advocating for residents to address any concerns or issues that may arise. The Admission Assistant must exhibit outstanding organizational skills, a collaborative spirit, and a proactive approach to problem-solving.

Job Duties

  • Participates in initial admission screenings, identifying needs, and meeting with family members
  • Acts as a liaison and resident advocate by addressing any issues and concerns among residents, families, staff and outside resources
  • Coordinates individual and family meetings as needed or prescribed in the plan of care
  • Maintains appropriate documentation in the clinical and business records per ministry policy
  • Performs other duties as assigned
