Job Requirements
- High school diploma or equivalent
- Minimum of one year experience working with a frail or elderly population preferred
- 3-5 years administrative and clerical experience preferred
- Current and unrestricted driver’s license preferred
- Strong collaborative and interpersonal skills
- Ability to manage multiple deliverables concurrently
- Proficiency with Microsoft Office
- Must demonstrate integrity and personal commitment
- Ability to function in a matrix environment
- Excellent verbal and written communication skills
Job Qualifications

Job Duties
- Participates in initial admission screenings, identifying needs, and meeting with family members
- Acts as a liaison and resident advocate by addressing any issues and concerns among residents, families, staff and outside resources
- Coordinates individual and family meetings as needed or prescribed in the plan of care
- Maintains appropriate documentation in the clinical and business records per ministry policy
- Performs other duties as assigned
