Job Requirements
- Bachelor's degree in recreational therapy or related field
- Minimum of two years of recent experience working with frail or elderly populations
- CPR and First-Aid certification or ability to obtain within six months
- Obtaining Activity Coordinator State certification within first ninety days of hire
- Bilingual proficiency in English and Spanish or Vietnamese
- Ability to work a flexible schedule including evenings and weekends
- Capacity to manage multiple projects effectively
- Strong communication and interpersonal skills
- Proficiency in Microsoft Office applications
- Must work full office with potential limited telework within California
Job Qualifications
- Bachelor's degree in recreational therapy or a related field
- Two years of experience providing services for the frail or elderly population within the last five years
- CPR and First-Aid certification or ability to obtain within six months of hire
- Activity Coordinator State certification obtained within the first ninety days of hire
- Bilingual in English and either Spanish or Vietnamese
- Experience in event planning or activity coordination is a plus
- Experience designing engaging activities for diverse groups with different interests and abilities is desirable
- Ability to develop rapport and establish effective working relationships with diverse teams and external contacts
- Strong independent judgment and sound decision-making skills
- Excellent oral and written communication skills
- Flexibility to work evenings and weekends
- Strong organizational, analytical, and project management skills
- Ability to manage multiple projects and facilitate collaboration across teams
- Leadership skills to motivate multi-program teams and external committees
- Proficiency with Microsoft Office and job-specific software
Job Duties
- Participates in a mission-driven culture of high-quality performance with a member focus on customer service, consistency, dignity and accountability
- Assists the team in carrying out department responsibilities and collaborates with others to support short- and long-term goals/priorities for the department
- Coordinates, implements and oversees the activity program, updates the activity calendar and works with various departments to ensure entertainment and classes are scheduled
- Coordinates the day center's social programming and activities, including communicating current events
- Collaborates with supervisor to oversee the day floor activities
- Conducts comprehensive, semi-annual, and periodic assessments for all participants
- Establishes attainable and measurable goals and schedules individualized recreational activities and social activities as part of the plan of care
- Communicates participant changes to team members and reports concerns to the Interdisciplinary Team (IDT)
- Records, signs and dates progress reports in participants' medical records
- Explains activities and programming to participants and works with therapists and primary care staff to enhance the day center experience of participants
- Builds relationships and coordinates with community resources, including organizing performances and holiday celebrations
- Coordinates recreational and activity outings in the community where possible
- Participates in Quality Assurance (QA) activities
- leads participant discussions on PACE program improvements
- Serves as a member of the IDT and attends staff meetings and participates in participant care planning
- Completes other projects and duties as assigned
