Job Description

Clover Adult Day Care Program, along with Heritage Senior Residences at Columbia Parc and the residents at Sacred Heart, represents a compassionate and dedicated community focused on providing comprehensive care and enriching experiences for elderly and disabled adults. The Activities Coordinator role is a critical position within Clover, designed to plan, organize, and implement a well-rounded program of activities aimed at meeting the psychological, social, spiritual, emotional, and physical needs of participants. This role spans multiple facilities, including the Adult Day Care Program and Heritage Senior Residences, and extends services to residents at Sacred Heart. An effective Activities Coordinator enhances participants' quality of life by encouraging engagement in various leisure and therapeutic activities that promote mental stimulation, relaxation, physical exercise, and social interaction.

In this role, the Activities Coordinator is tasked with developing accessible and appealing activity calendars that address the diverse interests and capabilities of men and women across varying ages and physical abilities. They are responsible for fostering a lively and inclusive community by integrating onsite and offsite activities that encourage participation and connection. Additionally, the coordinator maintains meticulous records for all programs and participants, ensuring that objectives outlined in individual care plans are tracked and achieved.

Integral to the role is liaising between participants, families, community organizations, and program staff to create a connected support system. This involves coordinating volunteer programs that include recruitment, training, and recognition efforts to sustain a robust support network. The Activities Coordinator also plays a part in resident advocacy by facilitating Resident Council meetings and channeling feedback to appropriate care personnel, thereby empowering participants to voice concerns and influence their living environment.

Given the sensitive nature of working with elderly and disabled populations, the role requires strong interpersonal skills, empathy, and a thorough understanding of human behavior. The coordinator must demonstrate an ability to work under pressure, manage multiple deadlines efficiently, and uphold strict confidentiality regarding participant and agency information. The position demands proficiency with computer tools such as Microsoft Word, Excel, PowerPoint, Outlook, and Internet Explorer to assist in documentation and communication tasks.

Candidates should bring a blend of educational qualifications, preferably a Bachelor’s degree in Therapeutic Recreation, Social Work, Nursing, or a related field, along with hands-on experience in serving frail elderly or physically/mentally disabled populations. A proactive attitude toward continuous improvement, cultural sensitivity, and a strengths-based empowerment philosophy are essential to succeed in this dynamic and rewarding role. Job Requirements

  • Any combination of education and experience equivalent to a Bachelor’s degree in Therapeutic Recreation Social Work Nursing or related field
  • At least six months experience working with frail elderly or physically mentally disabled population preferred
  • Excellent creative and communication skills
  • Computer literate with MS Word Excel PowerPoint Outlook and Internet Explorer
  • Good knowledge of human behavior particularly in adults elderly participants

Job Duties

  • Develop organize and implement a program of activities to meet the social emotional physical and other therapeutic needs of residents
  • Initiate and promote activities both within and outside the facility as weather permits insuring the safety and wellbeing of each resident at all times
  • Coordinate and verify that assistance is provided to residents for activities and events as necessary
  • Create an ongoing calendar of activities which creatively meets multiple needs and is accessible and appealing to both men and women of all ages and abilities
  • Consistently maintain standards for activity scheduling and documentation established by policies and regulatory requirements
  • Solicit the involvement of the community when planning facility activities and events
  • Maintain detailed records of activity programs and participation of individual residents identifying progress toward established care plan goals
  • Participate in resident care planning by identifying the activity needs of residents in accordance with an assessment of needs and mobility
  • Develop and distribute a monthly calendar of events providing a copy in each resident’s room and a prominently displayed calendar in the facility
  • Maintain a pictorial scrapbook of activities and events sponsored by the facility
  • Develop and maintain an active volunteer program including marketing development support and organization
  • Train volunteers and students in techniques of recreation and therapy
  • Maintain all activity related records required by regulations and Medical Records including activity assessments progress notes and discharge summary
  • Assist residents with organizing and maintaining active Resident Council including accommodation assistance and scheduling
  • Operate between programs including Adult Day Care Services Heritage Senior Residences at Columbia Parc and Sacred Heart at St Bernard
  • Serve as a liaison between the participant program family and community assisting with community placements and resources when appropriate
  • Be familiar with safety procedures including Standard Precautions Fire Drill and Evacuation Plans
  • Maintain confidentiality of resident and facility records and information
  • Perform other tasks as assigned
