Job Description
The WISH IntakeDischarge Peer Specialist position is an essential role within a health and human services organization that operates in Multnomah and Clackamas Counties. Job Requirements
- High school diploma or equivalent
- prior experience in peer support or behavioral health services
- ability to communicate effectively and build rapport with clients
- knowledge of cultural competency and motivational interviewing
- availability to work full-time Monday through Friday
- ability to travel locally with clients as needed
- capacity to attend regular clinical supervision sessions
- competency in basic paperwork and documentation
- commitment to client confidentiality and ethical standards
Job Qualifications

- knowledge of person-centered planning and motivational interviewing techniques
- ability to build trust and rapport with diverse client populations
- experience collaborating with community-based agencies
- strong communication and interpersonal skills
- ability to manage a reduced caseload of clients
- familiarity with cultural competency principles
Job Duties
- Apply person-centered principles to build relationships, trust, and rapport with clients
- complete WISH assessments and develop goal action plans accordingly
- prepare discharge resources specific for each client, working with peer to assess individual client needs
- process initial paperwork for transition out of WISH and track pre-auth for continued, non-WISH acupuncture services
- utilize cultural competency, active listening, and motivational interviewing skills to assist clients in reaching their treatment goals
- establish relationships with local agencies
- collaborate with local agencies to coordinate services
- carry a reduced caseload of peer mentorship
- travel with clients to and from appointments, as needed
- assist clients in carrying out the activities to fulfill their goal plans and problem solve with clients to overcome challenges
- attend regular clinical supervision as required per certification
- facilitate groups as assigned by Coordinator
- complete required reports quarterly
- work closely with internal and local Peer Support Specialists and Certified Recovery Mentors to maximize the quality of community service
- attend regular clinical supervision meetings
