Job Requirements

  • High school diploma or general education degree (GED)
  • one to two years related experience and/or training or equivalent combination of education and experience
  • knowledge of medical terminology preferred
  • computer proficiency or ability to easily learn required
  • current driver's license and automobile insurance as required by Tennessee State Law

Job Duties

  • Maintain the hospice aide schedules
  • update schedules when patient status changes
  • create schedules for two-weeks out from current date
  • add new patients as directed by RN/CM verifying visit frequencies and Hospice Aide plan of care documented in Electronic Medical Record
  • ensure plans of Care (PPOC) and Certification of Terminal Illness (CTI) are signed by attending physician and/or medical director and print/send for signature if physician is a non-employee
  • verify all necessary elements completed on Election of Hospice Benefit (EHB)
  • scan required documents into EMR as indicated
  • notify all physicians and referral sources of patient death and document notifications in EMR
  • verify EHB and POST form (if applicable) in EMR when patient is transferred to inpatient unit (IPU)
  • send care plan updates biweekly to attending physician throughout patient stay
  • print and send orders to appropriate physician for signature for home care
  • forward discharge summaries to attending physician for live discharges
  • prepare and distribute interdisciplinary team (IDT) meeting agenda
  • copy and distribute forms and materials to IDT members
  • utilize copier/scanner, fax, and phone system
  • order office supplies and forms for the team
  • maintain files of active patient records
  • perform clerical duties for Director Clinical Care and IDT
  • enter data and obtain reports from EMR
  • scan and index patient documents to EMR daily
  • print, fax, and register returned signed orders in EMR, and follow up on unsigned orders
  • manage computer and projector for patient charts during meetings
  • prepare and mail bereavement cards
  • prepare recertification templates for physicians
  • perform receptionist duties as requested
  • perform other assigned duties
