The role available is a Registered Nurse (RN) specializing in home health care, where the incumbent will engage in providing clinical services under the scope of nursing practice as defined by state laws. This position involves working closely with a multidisciplinary healthcare team and executing care plans tailored to individual patient needs. The RN will play a pivotal role in managing referrals, ensuring proper clinician assignments, evaluating patient orders, coordinating insurance approvals, and continuously assessing patient conditions to ensure delivery of appropriate and effective home health care services. This is a full-time position offering an annual salary ranging from $72,800 to $130,000, reflecting factors such as location, education, experience, and certifications. Job Requirements

  • current unrestricted RN licensure in state of practice
  • current driver's license
  • vehicle insurance
  • access to a dependable vehicle or public transportation
  • ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Job Qualifications

  • current unrestricted RN licensure in state of practice
  • ability to function in any home situation regardless of patient demographics
  • home care experience preferred
  • current CPR certification preferred
  • able to work independently
  • good communication, writing, and organizational skills

Job Duties

  • provides clinical services within the scope of nursing as per state laws and plan of care
  • receives referrals and ensures appropriate clinician assignments
  • evaluates patient orders and schedules start of care visits
  • coordinates determination of patient home health benefits and insurance approvals
  • continuously assesses patient needs and ensures care is individualized, appropriate, and meets eligibility criteria
  • reviews assessments and plans of care daily and consults clinicians with recommendations
