The Patient Care Facilitator (PCF) role at North Memorial Health is a critical nursing position specifically tailored for the NICU environment at Maple Grove Hospital. The PCF works proactively to ensure seamless customer care transition from shift to shift, focusing on patient care coordination, quality, and the satisfaction of patients and their families. This role is instrumental in resource allocation by managing appropriate patient assignments, staffing, and scheduling, while also providing direct newborn and family care when required. PCFs demonstrate the mission and values of Maple Grove Hospital consistently in daily operations. The NICU-specific responsibilities include reviewing maternal prenatal history, monitoring newborn health, coordinating care plans with healthcare teams, conducting physical and psychosocial assessments during transitions such as admission and discharge, recognizing risk factors and intervening appropriately, supporting basic life support until advanced providers arrive, administering medications, providing IV therapy, performing point of care testing, collaborating in multidisciplinary rounds, educating families on newborn care and rehabilitation, and maintaining thorough and accurate documentation in electronic medical records such as Epic. Candidates for this role are expected to be experienced registered nurses with 3-5 years’ nursing background in hospital NICU settings, with proficiency in electronic medical records and strong communication skills. The position is scheduled for 12-hour night shifts with weekend rotation every third weekend and offers a competitive salary range of $46.32 to $74.81 per hour depending on experience. Employment is on-site at the Maple Grove Hospital, and health insurance benefits are available. This role requires a current RN license in the state of Minnesota and current BLS certification.   • Ability to maintain composure in high stress situations
  • Strong PC skills including electronic medical records and MS applications
  • Effective communication skills both written and oral
  • Ability to deal with conflict positively
  • Accountability for quality customer service
  • Ability to maintain quality, safety and infection control standards
  • Current RN licensure in Minnesota
  • Current BLS certification
  • Must be able to read, write and speak English effectively
  • Ability to work 12-hour night shifts and every third weekend
  • Employment authorization to work full-time in the United States
  • On-site work availability.

Job Qualifications

  • Associates Degree in nursing
  • Bachelor’s Degree in nursing preferred
  • 3-5 years nursing experience in hospital NICU as an RN
  • Demonstrated ability to maintain composure in high stress situations and act quickly in response to critical situations
  • Previous experience in care coordination and quality management activities preferred
  • Solid PC skills including electronic medical record and MS applications
  • Strong communication skills in written and oral form
  • Ability to handle conflict positively and consider the needs, feelings and capabilities of others
  • Accountability for quality customer service
  • Ability to maintain quality, safety and infection control standards
  • Ability to read, write and speak effectively in English
  • Current licensure and registration as an RN in the State of Minnesota
  • Current BLS certification for all nursing practice areas.

Job Duties

  • Review maternal prenatal history and collect current signs and symptoms of the newborn
  • Consult and coordinate with health care team members to assess, plan, implement and evaluate customer plan of care
  • Perform transition, admission, discharge and transfer assessments considering both physical and psychosocial elements of newborn and family
  • Recognize risk factors, change in status and interventions appropriate for newborn care
  • Initiate corrective action whenever information from monitoring equipment shows adverse symptomatology of the newborn
  • Manage basic life support needs and stabilize newborn until physician or neonatal nurse practitioner is available
  • Record customer’s vital signs, assessments, patient care and interventions as ordered
  • Work directly with physicians and neonatal nurse practitioners assisting during exams, diagnostic testing, treatments and interventions
  • Participate and collaborate in multidisciplinary rounds
  • Prepare and administer medications (orally, subcutaneously, IM and IV) and record prescribed medications
  • Report adverse reactions to medications or treatments
  • Demonstrate phlebotomy skills
  • Provide IV therapy
  • Provide bedside care
  • Perform point of care testing
  • Operate, maintain and discontinue proper supplies and appropriate medical equipment for customer care
  • Report any suspected abuse to appropriate agencies
  • Initiate family education plan as prescribed by physician and teach families how to manage newborn care
  • Display professionalism while completing multiple urgent tasks timely
  • Record all care information concisely, accurately and completely in a timely manner in Epic.
