The RN Manager - Care Management Social Work role at Providence Portland Medical Center is a full-time, day-shift leadership position. This role plays a crucial part in the management and direction of the inpatient Care Management Department, overseeing a team of approximately 24-31 full-time employees and managing a substantial departmental budget between $2.4 million and $2.9 million. The Care Management Manager ensures the excellence and advancement of the clinical care management model, facilitating the coordination and quality of patient care across multidisciplinary teams. This leadership role involves working closely with a diverse group of stakeholders, including medical staff, facility executives, finance departments, utilization management professionals, and community partners such as health plans and coordinated care organizations. The manager’s responsibilities extend to establishing efficient discharge planning mechanisms and innovating care transition models that support successful patient reintegration into home and community settings.
The position offers an annual incentive program and a highly competitive paid leadership time off structure, affirming Providence’s commitment to rewarding leadership excellence. The pay range for this role is from $65.24 to $102.99 per hour, with potential additional compensation such as shift differentials, standby/on-call pay, overtime, premiums, extra shift incentives, or bonus opportunities. Job Requirements
- bachelor’s degree in nursing
- oregon registered nurse license upon hire
- 2 years of supervisory experience including program planning, performance management, staff development and evaluation
- 1 year of care management experience
- 3 years of relevant experience in social services, utilization review or quality management, or case management
