The role of the Pool RN Case Manager is a critical position within Baptist Health’s interdisciplinary care teams. This position supports physicians and the broader medical team in facilitating superior patient care by integrating case management and discharge planning functions. The primary objective is to enhance clinical outcomes and patient satisfaction while managing healthcare costs effectively. The position requires managing a designated patient caseload, planning care efficiently to meet patient needs, controlling the length of stay, and promoting optimal resource utilization.

Key responsibilities include collaborating across care teams to oversee patient care transitions, intervening to address barriers that may delay care or reimbursement, applying process improvement methodologies to evaluate care outcomes, coordinating communication among healthcare providers, patients, families, and post-acute care services, and screening to prevent hospital readmissions.

This full-time position offers a competitive hourly wage estimated at $47.00 per hour, varying by shift. Work schedules may range from standard daytime shifts to extended hours, ensuring coverage and continuous patient care. Job Requirements

  • Associate degree in nursing
  • Registered Nurse license
  • Completion of BSN within 3 years if hired after 2/2012
  • Minimum 3 years hospital clinical experience
  • Preferred 1-3 years hospital or payor case management experience
  • Excellent interpersonal communication skills
  • Strong analytical and computer skills
  • Knowledge of discharge planning and utilization management
  • Understanding of acute and post-acute care resources
  • Strong organizational skills
  • Ability to work independently and exercise sound judgment

Job Qualifications

  • Associate degree in nursing
  • Registered Nurse license
  • RNs hired prior to 2/2012 with an Associate degree not required to have a BSN to continue non-leadership RN role
  • Required to complete BSN within 3 years of hire
  • 3 years hospital clinical experience
  • 1-3 years hospital or payor case management experience preferred
  • Excellent interpersonal communication and negotiation skills
  • Strong analytical, data management, and computer skills
  • Current knowledge of discharge planning, utilization management, case management, and performance improvement preferred
  • Understanding of pre-acute and post-acute care venues and community resources preferred
  • Strong organizational and time management skills
  • Ability to work independently and exercise sound judgment in interactions with healthcare team and patients/families

Job Duties

  • Support physician and interdisciplinary team in facilitating patient care
  • Manage designated patient caseload and plan care to meet patient needs
  • Coordinate discharge planning and care facilitation functions
  • Apply process improvement methodologies in evaluating outcomes of care
  • Coordinate communication with healthcare team, patients, families, and post-acute care services
