Job Requirements
- Bachelor's degree in health care, public health, or related clinical field preferred
- Experience working in healthcare in a patient-facing role preferred
- Active BLS certification
- Valid Ohio driver's license and proof of auto insurance as required by hospital policy and position-specific requirements
- Must pass motor vehicle background inspection, insurance eligibility, driving qualifications, and training set forth by Nationwide Children's Hospital and maintain qualification of insurance guidelines
- Working knowledge of Medicaid and other regulatory agency standards

Job Duties
- Communicates with primary care physicians, specialty physicians, behavioral health physicians, community provider office staff, and other service providers to identify and implement outreach services
- Reports out monthly on the impact of outreach services utilizing tools created by the PFK data team, claims data, Epic, and data pulled from the community provider EMR
- Identifies innovative outreach opportunities
- collaborates with PFK team to design and pilot new outreach methods
- Collaborates with PFK Quality Improvement, Pharmacy Team, and Provider Relations on outreach efforts and other support opportunities associated with their project work
- Serves as a liaison with NCH marketing to design and order outreach materials
- Develops and maintains a standard collection of outreach materials
- Develops tracking resources to support member identification and outreach impact
