Job Description

InnovAge PACE (Program of All-inclusive Care for the Elderly) is a pioneering healthcare organization focused on providing an alternative to traditional nursing home care. The program is designed to support elderly individuals by delivering personalized healthcare services, social engagement opportunities, and physical exercise, all aimed at helping older adults maintain their independence and stay in their homes longer. InnovAge PACE operates community-based centers where participants receive the majority of their healthcare and social support services within a comforting and familiar environment. The holistic team-based approach combines the expertise of healthcare professionals and personal care plans tailored specifically to each participant's unique needs, ensuring comprehensive and compassionate care.

The role of Behavioral Healthcare Provider (BHP) at InnovAge PACE is critical to the success of this model of care. This position is a Monday through Friday, daytime role with no on-call or weekend hours, providing a desirable work-life balance. The Behavioral Healthcare Provider plays an integral role in both developing and executing individualized care plans, ensuring that behavioral health services are delivered with high-quality, clinical precision, and a team-oriented approach. Primary responsibilities include conducting initial behavioral health assessments, providing therapeutic interventions, managing referrals for participants and families, and making treatment recommendations to support optimal outcomes. Physical presence at an InnovAge center is essential, as is the ability to travel to meet participants in other community settings when necessary.

This role demands a professional who thrives in a dynamic and evolving care environment, one who can confidently navigate ambiguity, multitask efficiently, and adapt interpersonal approaches to meet the real-time clinical needs of the participants and interdisciplinary healthcare team (IDT). The provider will utilize evidence-based behavioral therapies, including Motivational Interviewing, Solution-Focused Brief Therapy, Dialectical Behavioral Therapy, Acceptance Commitment Therapy, and Cognitive Behavioral Therapy. Openness to feedback and reflective practice is essential, as is a commitment to continuous professional development and collaborative care.

The Behavioral Healthcare Provider’s work is roughly split between clinical care (70%) and administrative duties (30%). Clinically, the provider completes mental health evaluations, documents findings in the electronic medical record (EMR), delivers solution-focused interventions ranging from monitoring to crisis management, consults with team members, and coordinates with external psychiatric providers to stabilize at-risk participants. In terms of administration, they educate the interdisciplinary team to address behavioral complexities, liaise with external providers to ensure continuity of care, and actively participate in relevant team meetings and committees. This mix of responsibilities ensures that the Behavioral Healthcare Provider not only delivers direct patient interventions but also contributes to the overall care coordination and quality improvement efforts of the program.

InnovAge PACE is looking for licensed mental health professionals with at least a Master's degree in behavioral health, social work, or psychology and a minimum of one year’s experience in an integrated care setting. Accepted licenses include LCSW (Licensed Clinical Social Worker), Licensed Psychologist (PsyD or PhD), LMFT (Licensed Marriage Family Therapist), or LPC (Licensed Professional Counselor). Current CPR certification or the ability to obtain certification within the first 30 days of employment is also required.

Preferred qualifications include experience working with geropsychiatric populations, training in group and family interventions, and expertise in conflict management, mediation, de-escalation, and crisis mitigation. Candidates with knowledge of chronic disease management's psychosocial impacts, bilingual abilities, doctoral degrees in clinical psychology, integrated primary care experience, or certifications in substance use treatment are highly encouraged to apply. Job Duties

  • Completes mental health assessments and documents in the EMR including diagnosis, recommendations, team-based care planning, and other regulatory requirements
  • provides solution-focused monitoring, therapeutic, or crisis intervention to participant and/or caregiver(s)
  • consults with team members regarding clinical findings, provides interventions, and participates in case conferencing
  • collaborates with interdisciplinary team, external providers, and caregivers to support participants at risk for inpatient psychiatric hospitalization including referral and coordination with outside resources
  • monitors response to psychotropic medications and coordinate with psychiatric consultant to ensure continuity of care
  • educates interdisciplinary team to manage and resolve behavioral complexities or placements
  • liaisons with external psychiatric providers to ensure appropriate referrals, continuity of care, monitoring, follow-up, and reporting back to the interdisciplinary team
  • participates in interdisciplinary team meetings, conferences, committees, and events as requested
