The PRN AgeWell Cincinnati Concierge Care Manager position at JFS offers an opportunity for experienced clinicians passionate about geriatric care to work in a flexible, client-centered role. This role is part of a private-pay geriatric care management practice and provides Care Management services on an as-needed PRN basis. The ideal candidate will be motivated to build a caseload while enjoying the benefit of a flexible schedule. This position requires conducting on-site visits in the Cincinnati area, ensuring that clients and their family caregivers receive comprehensive and personalized support.

In this role, the Concierge Care Manager will conduct thorough client assessments encompassing medical, psychosocial, functional, and environmental factors. They will develop individualized care plans in collaboration with clients and their families, ensuring that care is coordinated effectively across providers, community resources, and support services. Responsibility for maintaining accurate and timely client records—including clinical and billing documentation—is also key. The role demands strong communication skills to maintain regular contact through in-person visits, phone calls, and written communication with clients, caregivers, and other involved parties.

This position emphasizes advocacy and guidance during critical healthcare transitions and crises, facilitating family communication and care conferences, and providing education and coaching to both clients and their caregivers. The Concierge Care Manager acts as the primary point of contact for assigned clients, ensuring high levels of customer service and responsiveness.

The schedule is flexible and dependent on caseload size and client needs, with hourly compensation at $50 per hour, paid as a W-2 employee. This is a PRN role with no guaranteed hours or employee benefits, but individuals are eligible for paid agency-required trainings. Independence, excellent organizational and interpersonal skills, and a passion for improving the lives of older adults are essential for success in this rewarding position.

Job Requirements

  • Active, unrestricted professional license
  • Bachelor’s degree required
  • Minimum 2-5 years of care management or geriatrics experience
  • Strong knowledge of healthcare systems and care coordination
  • Experience working with families in crisis
  • Excellent communication and organizational skills
  • Valid Ohio driver’s license and current automobile insurance
  • Ability to work independently with minimal supervision

Job Qualifications

  • Minimum 2-5 years of care management or geriatrics experience

  • Excellent interpersonal communication and organizational skills
  • Ability to work independently with minimal supervision
  • Valid Ohio driver’s license and proof of current automobile insurance

Job Duties
