Primary Care Provider, Nurse Practitioner / Physician Assistant

The role of Primary Care Provider (PCP) at WelbeHealth is central to this mission. Reporting to the Medical Director, the PCP plays a critical role in forming lasting relationships with participants by serving a smaller patient caseload, which allows for true value-based care. This position emphasizes collaboration with physicians, therapists, and other members of the IDT to create holistic care plans and educate both participants and their families about health management. The Primary Care Provider will be responsible for conducting comprehensive in-person assessments, including initial, semi-annual, and unscheduled evaluations, to ensure participants’ medical and care needs are continually met. They will coordinate discharge planning from acute or long-term care settings and oversee care in skilled nursing facilities and residences. PCPs will also review outcomes and utilization data such as hospitalizations and specialist use to identify best practices and improve policies.
