This particular role is designed to provide essential supportive household and personal care services to members following prescribed care plans. The position focuses on light housekeeping, meal preparation including medically prescribed diets, laundry, grocery shopping, and other household duties integral to maintaining a healthy and safe living environment for clients. The caregiver will actively monitor and report changes in the client's physical, mental, and social statuses to supervisors and agency staff to ensure timely adjustments to care plans. This position requires close adherence to service plan goals, and although prior experience as a sitter, companion, or homemaker is considered a plus, it is not mandatory, making this an excellent opportunity for individuals seeking entry into the healthcare support field.

Key responsibilities include assisting with personal care as directed by service plans, helping with advanced supportive restorative (ASR) tasks upon training from licensed nurses, and maintaining comprehensive documentation of all observations and activities related to care. The role demands physical activity, including standing, walking, and lifting objects weighing between 10 to 40 pounds. Travel between members' homes and running errands is also expected.

Employees in this role must exhibit excellent verbal and written communication skills to effectively interact with scheduling coordinators and various members while safeguarding confidentiality. The position involves extensive communication through telephone, electronic media, or face-to-face engagement. Active participation in agency activities is required, such as attending staff meetings, completing at least twelve hours of in-service training annually, and fulfilling the care hours stipulated in the care plan.

Job Duties
