Job Requirements

  • Be legally authorized to practice in the state of employment
  • Act only within the scope of authority to practice
  • Minimum 1 year experience working with frail or elderly populations
  • Meet standardized competencies for the position before working independently
  • Medically cleared for communicable diseases before engaging in direct participant contact
  • Associates or vocational degree or equivalent work experience
  • CPR (Basic Life Support) certification
  • Proficiency in relevant computer software
  • Strong communication and organizational skills
  • Ability to work independently
  • Compliance with HIPAA laws and privacy policies
  • Ability to lift, push, pull, or carry up to 35 pounds occasionally

Job Qualifications

  • Associates or vocational degree in therapeutic recreation, hospitality, healthcare, or related field, or equivalent relevant work experience
  • Minimum of 2 years experience in event planning, recreation, or activities with a focus on geriatric or frail populations
  • CPR (Basic Life Support) certification
  • Demonstrated ability to work effectively with individuals with various health issues, concerns, and personalities
  • Excellent organizational skills and proficiency in computer applications including Word, Excel, PowerPoint, and Outlook
  • Strong verbal and written communication skills with active listening abilities
  • Ability to document case notes, assessments, contacts, and write letters that may be part of legal records
  • Capacity to work independently with minimal supervision

Job Duties

  • Plan, organize, and lead games, arts and crafts, social activities, arts and theater, and therapeutic activities
  • Train and oversee activity aides in day centers, delegate assignments, and oversee performance
  • Serve as a member of the PACE Interdisciplinary team and conduct assessments to determine participants' needs and interests, evaluate effectiveness of activities, and participate in development of interdisciplinary Plans of Care
  • Accurately document in the Electronic Health Record (EHR) in a timely manner
  • Provide health education, training, consultation, and support to aides, participants, and family caregivers
  • Encourage and motivate participants to engage in activities, providing support and companionship
  • Collaborate closely with other healthcare professionals, including doctors, nurses, and social workers, to provide integrated care
  • Assist with personal care of participants as needed, including escorting participants to onsite facilities, restrooms, and providing assistance with eating
  • Plan and execute varied monthly programs using group and individual-oriented interventions to maximize participants' functional abilities
  • Accountable for planning and implementing community outings, special events, and activity programming goals
  • Participate in other tasks, committees, and special projects as needed
  • Actively participate in team discussions, offering feedback, ideas, and suggestions during department staff meetings
  • Manage supplies and resources needed for activities, staying within budget
  • Provide all services in a manner consistent with State and Federal regulatory requirements
  • Ensure all activities are conducted safely and in compliance with relevant regulations
  • Serve on various internal and external committees as required or designated
  • Participate in team meetings, caregiver groups, and special events as needed
  • Foster a safe and supportive environment conducive to the professional development of healthcare professionals
