Job Description

As a Banner Home Health Occupational Therapist Assistant, you will play a crucial role in delivering occupational therapy treatments in sub-assigned geographic territories including Phoenix, Mesa, Scottsdale, Tempe, Ahwatukee, Guadalupe, Chandler, and Gilbert. You will operate in a semi-autonomous environment characterized by supportive management and equipped with resources such as a company cell phone and tablet loaded with the electronic medical records system (Home Care Home Base) for efficient documentation. This role involves administering therapy treatments under the supervision of an Occupational Therapist, ensuring therapies are tailored to each patient's unique physical, social, and cultural contexts. You will be expected to comply with physician referrals, state and local regulations, and Banner Health's departmental policies.
The position offers a flexible schedule with training conducted Monday through Friday from 8:00 a.m. to 5:00 p.m. over 60 days, transitioning into a PRN (as-needed) role requiring the commitment to at least two shifts per month. You will be part of a reputable healthcare network that emphasizes patient safety, personalized treatment planning, and interdisciplinary collaboration. Job Requirements
- Must have graduated from an Occupational Therapy Assistant program accredited by the AOTA
- Must possess a current Occupational Therapy Assistant license from the state of practice
- Must have BLS certification
- Must possess Arizona Fingerprint Clearance Card at time of hire and maintain throughout employment
- Must demonstrate clinical competency and experience
- Must complete occupational health physical assessment and drug screen post-offer
- Must pass background check including employment, criminal, and education verification
Job Qualifications
- Graduated from an accredited Occupational Therapy Assistant program
- Possess current Occupational Therapy Assistant license for state of practice
- BLS certification
- Demonstrated clinical competency and experience
- Effective communication skills
- Ability to document accurately and timely
- Experience with electronic medical records preferred

Job Duties
- Administers occupational therapy treatment according to plan of care
