Job Requirements
- Current Wisconsin nursing assistant or home health aide certification
- Ability to provide reliable transportation
- Willingness to work on-call schedule primarily Monday through Friday
- Strong interpersonal and communication skills
- Ability to perform physical tasks associated with caregiving
- Flexibility to cover multiple locations including Twin Cities area and western Wisconsin
- Basic English proficiency including reading, writing, speaking, and understanding
Job Qualifications
- Successful completion of state-approved nursing assistant training program
- Current certification in applicable state(s)
- Competency in person-directed and elder/resident-directed care
- Skills in critical thinking, teamwork, self leadership, and interpersonal communication
- Ability to communicate effectively in English
- Demonstrated alignment with Presbyterian Homes & Services mission and values
Job Duties
- Provide personalized care to hospice clients in their homes
- Assist clients with activities of daily living and comfort measures
- Follow care plans developed by the interdisciplinary team
- Communicate effectively with supervising Registered Nurses and team members
- Maintain compliance with regulatory requirements and best practices
- Document care and client progress accurately
- Respond flexibly to assignments in various locations including Twin Cities and Wisconsin sites
