Job Requirements
- Master's degree or commensurate experience
- current RN and NP licensure with prescription authority or ability to obtain it
- board certification by AANP, ANCC, or AACCN
- current BLS, ACLS or CPR certification
- minimum 3 years experience as a Nurse Practitioner
- strong computer skills
- excellent communication skills
- ability to adapt to frequent changes
- willingness to travel extensively including overnight and weekends
- valid state driver's license
- ability to drive a car
- proof of adequate automobile insurance coverage
Job Qualifications
- Master's degree or commensurate experience and satisfactory completion of NP licensure
- current RN and NP licensure in state of practice with prescription authority or ability to obtain prescriptive authority
- board certification by AANP, ANCC, or AACCN in a Matrix approved specialty
- current BLS, ACLS or CPR certification
- minimum of 3 years experience as a Nurse Practitioner
- strong computer skills and familiarity with employee health/medical record software
- excellent verbal and written communication skills with patients, clients, and colleagues
- comfort and flexibility with frequent change
Job Duties
- Conduct adult/geriatric assessments
- review medical history, diagnosis and treatment
- provide health education
- make physician and case management referrals
- follow-up and document according to Matrix guidelines and protocols
- collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel
- provide patient education and coordinate follow-up with primary care physicians
- provide services in various settings including home visits and skilled nursing facility visits
