Job Requirements

  • Master's degree or commensurate experience
  • current RN and NP licensure with prescription authority or ability to obtain it
  • board certification by AANP, ANCC, or AACCN
  • current BLS, ACLS or CPR certification
  • minimum 3 years experience as a Nurse Practitioner
  • strong computer skills
  • excellent communication skills
  • ability to adapt to frequent changes
  • willingness to travel extensively including overnight and weekends
  • valid state driver's license
  • ability to drive a car
  • proof of adequate automobile insurance coverage

Job Qualifications

  • Master's degree or commensurate experience and satisfactory completion of NP licensure
  • current RN and NP licensure in state of practice with prescription authority or ability to obtain prescriptive authority
  • board certification by AANP, ANCC, or AACCN in a Matrix approved specialty
  • current BLS, ACLS or CPR certification
  • minimum of 3 years experience as a Nurse Practitioner
  • strong computer skills and familiarity with employee health/medical record software
  • excellent verbal and written communication skills with patients, clients, and colleagues
  • comfort and flexibility with frequent change

Job Duties

  • Conduct adult/geriatric assessments
  • review medical history, diagnosis and treatment
  • provide health education
  • make physician and case management referrals
  • follow-up and document according to Matrix guidelines and protocols
  • collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel
  • provide patient education and coordinate follow-up with primary care physicians
  • provide services in various settings including home visits and skilled nursing facility visits
