The role of Advanced Practice Clinician (APC), which includes Nurse Practitioners (NPs) and Physician Assistants (PAs), is essential to our HouseCalls program. This position is a non-prescribing, field-based role focusing on conducting comprehensive annual in-home health assessments for our Medicare Advantage and other plan members. These assessments encompass past medical history reviews, medication reconciliation, vital sign checks, physical exams, and evidence-based screenings. The APC collaborates closely with members’ PCPs by communicating findings, identifying urgent health situations, educating patients about disease processes, medications, and treatment adherence, and addressing social determinants of health impacting patient outcomes.

This role offers significant work-life balance benefits, including flexible scheduling options that allow providers to flex their hours between 8 a.m. and 8 p.m. over a seven-day week. Importantly, this position requires no on-call duties, no weekend shifts, and no holiday work, supporting personal time and family commitments.

We understand the value of rewarding top talent and therefore provide competitive compensation that generally ranges from $109,500 to $164,000, which includes base pay and potential bonuses. Job Requirements

  • Active, unrestricted licensure in the state you will be practicing in or ability to obtain by start date without current disciplinary actions or practice limitations
  • Active and unrestricted driver's license
  • Access to reliable transportation to complete home visits
  • Ability to navigate varied home environments
  • Ability to sit, stand, or kneel as needed to perform assessments
  • If working in a state that allows NPs/PAs to practice independently, approval from licensing board if required
  • New hires must apply for approval within designated timeframes if applicable

Job Qualifications

  • For nurse practitioners: active ANCC or AANP certification in family, adult, geriatric, or adult-gerontology primary care
  • For physician assistants: current NCCPA certification and state licensure
  • Proven communication skills with geriatric or Medicare populations
  • Proficiency with electronic medical records and technology
  • Ability to transport equipment weighing up to 30 pounds and navigate stairs as part of home visits

Job Duties

  • Conduct comprehensive in-home assessments including past medical history review, medication reconciliation, vital signs, and physical exam
  • Perform evidence-based screenings and point-of-care testing as appropriate
  • Identify and document diagnoses for care management and treatment planning
  • Communicate findings to members' primary care providers to address gaps in care
  • Recognize urgent and emergent situations and intervene appropriately
  • Educate members on disease processes, medications, and compliance
  • Address social determinants of health and provide referrals as needed
