The role of Advanced Practice Clinician (APC), which includes Nurse Practitioners (NPs) and Physician Assistants (PAs), is essential to our HouseCalls program. This position is a non-prescribing, field-based role focusing on conducting comprehensive annual in-home health assessments for our Medicare Advantage and other plan members. These assessments encompass past medical history reviews, medication reconciliation, vital sign checks, physical exams, and evidence-based screenings. The APC collaborates closely with members’ PCPs by communicating findings, identifying urgent health situations, educating patients about disease processes, medications, and treatment adherence, and addressing social determinants of health impacting patient outcomes.

This role offers significant work-life balance benefits, including flexible scheduling options that allow providers to flex their hours between 8 a.m. and 8 p.m. over a seven-day week. Importantly, this position requires no on-call duties, no weekend shifts, and no holiday work, supporting personal time and family commitments.

We understand the value of rewarding top talent and therefore provide competitive compensation that generally ranges from $109,500 to $164,000, which includes base pay and potential bonuses. In addition to salary, employees are eligible for a comprehensive benefits package that includes health insurance, a 401(k) plan with company contributions, employee stock purchase options, and various incentive and recognition programs. We also offer a significant signing bonus or student loan repayment option to support our team members’ financial well-being.
