The Clare Bridge Program Coordinator role at Brookdale offers an exciting opportunity for individuals passionate about making a positive impact on the lives of seniors living with dementia. Job Requirements

  • minimum high school diploma or equivalent
  • valid driver’s license and clean driving abstract as required
  • ability to handle physical demands such as standing, walking, sitting, lifting up to 50 pounds
  • ability to work occasional evenings, weekends or nights
  • ability to interact effectively with residents, coworkers, and vendors
  • willingness to comply with infection control and safety standards
  • must have the ability to plan and coordinate programs
  • must demonstrate cultural sensitivity and compassion towards elderly residents
  • ability to drive community vehicles if required

Job Qualifications

  • Bachelor’s degree in therapeutic recreation, gerontology, education or related field preferred
  • minimum two years of direct programming experience with older adults and persons with dementia preferred
  • valid driver’s license and current 3-year driving abstract with no more than one moving violation
  • knowledge of dementia care best practices and regulations
  • basic computer skills including typing and word processing
  • strong communication and interpersonal skills
  • ability to work independently and as part of a team

Job Duties

  • Designs a 7 day a week person centered program that is engaging, creative and meets the needs and interests of individual residents
  • develops a monthly calendar based on residents’ shared interests
  • ensures person centered special events and daily programs are scheduled and executed
  • ensures dementia care programs comply with Brookdale standards and regulations
  • collaborates with community leadership to educate and provide resources to families and associates
  • may supervise Clare Bridge Program Assistants including hiring, coaching, and counseling
  • participates in planning and coordinating special events
  • reports changes in residents’ health to clinical partners and collaborates on interventions
  • manages social media content to align with Brookdale brand
  • plans and schedules external programming events
  • maintains departmental budget and expense control
  • performs personal care, housekeeping, and dietary duties as needed
