As a bilingual (English/Cantonese) position, this role requires exceptional communication skills and the ability to advocate for patients with warmth and professionalism. The Care Facilitator manages patient check-ins and check-outs, schedules appointments with primary care physicians and specialists, and provides essential administrative support that ensures smooth clinic operations. This position is key to maintaining patient satisfaction by delivering superior customer service and fostering trusting relationships from the moment patients walk through the door. Responsibilities include managing schedules to optimize patient visits, accurately recording patient information, conducting outreach through phone calls and mailings to engage patients, and supporting healthcare teams with data reporting and operational coordination. The Care Facilitator also champions patient experience initiatives by soliciting feedback and addressing any concerns promptly. Job Requirements

  • High school diploma or equivalent education required
  • Minimum of 1 year work experience in a medical clinic or similar environment required
  • Spoken and written fluency in English and Cantonese
  • Ability and willingness to travel locally and regionally up to 10% of the time
  • Exceptional customer service skills and passion for serving others
  • Working knowledge of medical insurance and/or electronic medical record (EMR) systems
  • Skilled in operating phones, computers and basic IT systems
  • Ability to communicate professionally and courteously with patients and staff
  • Detail-oriented with strong verbal and written communication skills
  • Proficient in Microsoft Office Suite including Excel, Word and Outlook
  • BLS for Healthcare Providers preferred
  • Graduation from a nationally accredited Medical Assistant program preferred.

Job Duties

  • Interacts appropriately with patients and records information while greeting, checking-in, checking-out or scheduling appointments for patients
  • Conducts lobby checks to ensure organization and cleanliness
  • Serves as the gatekeeper for PCP and Specialists schedules ensuring Top 40 patients are scheduled every 13 days
  • Utilizes computer equipment and programs to add new patient information, prints out relevant forms, schedules appointments, responds to emails and generates New Patient packets
  • Conducts "love" calls and mails birthday and greeting cards to patients commemorating special events
  • Answers telephones, takes detailed and accurate messages, and directs messages to the appropriate individual
  • Clearly communicates information about ChenMed clinical personnel to patients and other individuals
  • Verifies patient insurance via phone or website and collects necessary copays for services
  • Assists with scheduling follow-up and in-house Specialist referral appointments
  • Escorts patients to exam rooms and ensures their comfort prior to leaving
  • Acts as HEDIS champion for the PCP by providing visibility of HEDIS needs and reporting on hospitalizations
  • Presents patients with customer service survey during check out and escalates if needed for immediate service recovery
  • Participates in daily and weekly huddles to provide patient details
  • Regulates and monitors aspects of transportation including driver’s lists, medication and grocery deliveries, driver behavior, and scheduling Uber/Lyft rides for patients
  • Maintains confidentiality of patient personal and medical information
  • Reviews patient personal and appointment information for accuracy
  • Transmits correspondence by mail, email or fax
  • Performs other duties as assigned by manager.
