The role of Manager, Care Team at Independent Living Systems is vital to the functioning and success of the care delivery process. This position oversees the daily operations of the care team, guiding a skilled group of healthcare professionals, caregivers, and administrative staff to ensure high standards in member care. The Manager is instrumental in developing and implementing individualized care plans that address the diverse needs of members, ensuring that each plan is both realistic and effective in promoting health and independence. In this supervisory role, the Manager provides ongoing support, training, and mentorship to team members, fostering an environment of professional growth and accountability.

Accountability for patient outcomes and satisfaction is a critical aspect of the role, with the Manager monitoring quality metrics and identifying areas for continuous improvement. Job Requirements

  • Bachelor's degree in Nursing, Health Administration, or a related field
  • Minimum of 5 years of experience in a healthcare setting
  • At least 2 years in a supervisory role
  • Residency in California
  • Current and valid California Driver's License
  • Use of personal vehicle with current registration
  • Proof of auto insurance meeting California minimum coverage
  • BCLS CPR Certification

Job Qualifications

  • Bachelor's degree in Nursing, Health Administration, or a related field
  • Minimum of 5 years of experience in a healthcare setting with at least 2 years in a supervisory role
  • Current nursing license or relevant certification preferred
  • Knowledge of and experience working with community agencies and programs
  • Experience with Medi-Cal eligibility guidelines, application, and renewal/redetermination process
  • Strong problem-solving and customer service skills
  • Residency in California with a current and valid California Driver's License
  • Use of personal vehicle with current registration
  • Proof of auto insurance meeting California minimum coverage
  • BCLS CPR Certification
  • Master’s degree in Social Work, Nursing, Health Administration, or a related field preferred
  • Experience with electronic health record (EHR) systems preferred

Job Duties

  • Conducts interviews and hiring of direct reports
  • Completes a ninety-day introductory and annual performance evaluation of each direct report
  • Facilitates professional growth and improved proficiency of staff who are direct and indirect reports through mentoring and training
  • Conducts ongoing performance management of each direct report
  • establishes performance goals

  • Ensure the development and implementation of care plans in collaboration with the care team, healthcare professionals, caregivers, and members
  • Coordinate daily operations of the care team, including scheduling, resource allocation, and workflow management
  • Monitor patient care quality metrics and implement improvement plans to address any gaps or challenges
  • Collaborate with healthcare providers, administrative staff, and external partners to ensure seamless care coordination
  • Manage compliance with healthcare regulations, organizational policies, and safety standards
  • Conduct one on ones, audits and regular team meetings to support professional growth and accountability
  • Develop and implement training programs to enhance team skills and knowledge
  • Address patient and staff concerns promptly and effectively to maintain a positive care environment
