We are currently recruiting for a hospitality-focused Licensed Practical Nurse (LPN) to join our talented team within the Memory Care Department. This full-time position entails working the night shift from 11:00 PM to 7:00 AM, with an hourly wage ranging from $37.42 to $46.61, plus an additional $3 shift differential. The role of an LPN at THE FORUM is integral to the well-being of our residents, providing direct nursing care and supervising our Quality of Life Specialists and Resident Assistants to ensure the highest standards of care are delivered.
As an LPN, you will be instrumental in supporting residents with their activities of daily living (ADL), overseeing medical and behavioral assessments, and collaborating closely with health services leadership. Your responsibilities include identifying changes in resident conditions, developing tailored care plans, and ensuring nursing care aligns with physician orders and established nursing practices. Job Duties
- Assist residents with activities of daily living (ADL), family/social activities and actively promote independent lifestyle of residents
- Responsible for identification of resident medical and behavioral changes and the development and monitoring of care plans and re-assessment of each resident
- Provide daily report to the Director of Health Services concerning community conditions, personnel performance, resident conditions, and needs for nursing supplies
- Provide nursing care according to physician’s orders and in conformance with recognized nursing practice, established standards, and administrative policies
- Ensure preventative, therapeutic and rehabilitative services to residents and assists in the coordination of nursing care with care rendered by other disciplines
