This position as a Hospice Registered Nurse (RN) or RN Case Manager is an integral role within Gentiva Hospice’s care delivery model. The role offers a competitive salary range between $118,000 and $120,000 along with a sign-on bonus available for those with direct hospice experience. The Hospice RN is the primary point of contact for patients and their families, guiding them compassionately through each phase of their hospice care. This involves delivering skilled, hands-on nursing care based on individualized Plans of Care, performing initial and ongoing assessments to monitor conditions, and adjusting care as needed. The RN works collaboratively with a multidisciplinary team to ensure that every aspect of the patient's needs—physical, emotional, spiritual, and practical—is addressed with sensitivity and professionalism.

Additionally, the RN educates caregivers and patients on medications, pain management, and the hospice philosophy to empower families with knowledge and confidence. Accurate clinical documentation, adherence to infection control and compliance standards, and communication of updates across the care team are key responsibilities for maintaining high-quality, consistent care. The role also supports quality improvement initiatives and scheduling efficiency. Job Requirements

  • Active RN license in the state of employment or eligible to obtain
  • Minimum 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields
  • Compassionate and patient-centered attitude
  • Comfortable providing care in private homes and facilities
  • Strong communication skills
  • Valid driver's license, auto insurance, and reliable transportation
  • CPR certification

Job Qualifications

  • Active RN license in the state of employment or eligible to obtain
  • Minimum 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields

  • Comfortable providing care in diverse environments including private homes and care facilities
  • Strong communication and critical thinking skills in emotionally sensitive situations
  • Valid driver's license, auto insurance, and reliable transportation
  • CPR certification required
  • Preferred experience includes hospital RN (Med-Surg, ICU, ER, PACU, telemetry, cardiac), admissions RN, wound care, float pool, travel nursing, experience with terminally ill or serious illness support

Job Duties

  • Be the primary point of contact for patients and families, guiding them through every step of their hospice journey
  • Deliver and document skilled, hands-on nursing care based on each patient's individualized Plan of Care in home, assisted living, or other settings
  • Perform initial and ongoing assessments to monitor patient condition and adjust care as needed
  • Collaborate with interdisciplinary team including physicians, LPNs, CNAs, social workers, chaplains to build and refine personalized care plans
  • Provide emotional, physical, and spiritual support to patients and their loved ones including education, comfort, and bereavement guidance
  • Teach caregivers how to care for their loved one safely and confidently with practical training and emotional reassurance
  • Communicate important updates in patient condition to the care team ensuring timely interventions
  • Maintain accurate, up-to-date clinical documentation following infection control and compliance standards
  • Support quality improvement and scheduling initiatives to ensure consistent, compassionate, and efficient care
  • Educate patients and families on medication administration, hospice philosophy, symptom and pain management, and end-of-life processes
