The role of Hospice RN / RN Case Manager at Gentiva Hospice is vital to ensuring high-quality, compassionate care is delivered to patients facing life-limiting illnesses. In this position, nurses become the primary point of contact for patients and families, guiding them through their hospice journey with empathy and professionalism. The RN performs skilled, hands-on nursing care, strictly adhering to individualized Plans of Care to manage symptoms, provide comfort, and support both physical and emotional needs. This role entails conducting initial and ongoing assessments to monitor patients’ conditions, making necessary adjustments to care plans in coordination with an interdisciplinary team that includes physicians, LPNs, CNAs, social workers, and chaplains. The Hospice RN also provides essential education and emotional support to both patients and their loved ones, including caregiver training and bereavement guidance, ensuring families feel confident and supported throughout the process.

This position requires meticulous clinical documentation and compliance with infection control standards, contributing to the organization's quality improvement initiatives. The RN also communicates key updates regarding patient condition to the wider care team to enable timely and appropriate interventions. The role addresses important topics such as medication administration, hospice philosophy and services, symptom and pain management, and end-of-life expectations.
