Job Requirements
- high school diploma or equivalent
- certified nursing assistant (cna) license
- current cpr certification
- six months of patient care experience preferred
- current driver's license and automobile insurance
- ability to comply with state-specific regulations
- strong interpersonal and communication skills
- ability to handle sensitive situations with empathy and professionalism
Job Qualifications

- six months of patient care experience in hospice, home health, icu, geriatric, or oncology strongly preferred
- current cpr certification
- knowledge of infection control measures and standard precautions
- ability to provide basic emotional and psychological support to patients and family members
- proficient in documentation and communication skills
- flexible and compassionate in providing care in various home environments
Job Duties
- report directly to the administrator, executive director or patient care manager senior and function under the direction, instruction and supervision of the registered nurse and/or clinical supervisor
- help the patient to maintain good personal hygiene
- assist in maintaining a safe and healthy environment
- prepare and serve light meals
- assist in feeding patient when needed
- assist the patient in ambulation
- assist with the safe use of a walker or wheelchair as taught and supervised by the nurse
- assist with light housekeeping tasks such as emptying trash and changing linens
- assist with certain treatments as ordered on the aide plan of care and as allowable in state-specific regulations
- attempt to promote patient's mental alertness through involvement in activities of interest
- give basic emotional and psychological support to the patient and other members of the household
- report any change in the patient's mental or physical condition or home situation to the registered nurse and members of the interdisciplinary team
- demonstrate basic care to the primary caregiver
- practice infection control measures and standard precautions
- provide accurate documentation with visit itineraries
- attend staff meetings
- participate in at least one quality improvement activity yearly, as assigned by supervisor
- complete documentation in a timely manner as outlined in orientation
