Job Requirements

  • high school diploma or equivalent
  • certified nursing assistant (cna) license
  • current cpr certification
  • six months of patient care experience preferred
  • current driver's license and automobile insurance
  • ability to comply with state-specific regulations
  • strong interpersonal and communication skills
  • ability to handle sensitive situations with empathy and professionalism

Job Qualifications

  • six months of patient care experience in hospice, home health, icu, geriatric, or oncology strongly preferred
  • current cpr certification
  • knowledge of infection control measures and standard precautions
  • ability to provide basic emotional and psychological support to patients and family members
  • proficient in documentation and communication skills
  • flexible and compassionate in providing care in various home environments

Job Duties

  • report directly to the administrator, executive director or patient care manager senior and function under the direction, instruction and supervision of the registered nurse and/or clinical supervisor
  • help the patient to maintain good personal hygiene
  • assist in maintaining a safe and healthy environment
  • prepare and serve light meals
  • assist in feeding patient when needed
  • assist the patient in ambulation
  • assist with the safe use of a walker or wheelchair as taught and supervised by the nurse
  • assist with light housekeeping tasks such as emptying trash and changing linens
  • assist with certain treatments as ordered on the aide plan of care and as allowable in state-specific regulations
  • attempt to promote patient's mental alertness through involvement in activities of interest
  • give basic emotional and psychological support to the patient and other members of the household
  • report any change in the patient's mental or physical condition or home situation to the registered nurse and members of the interdisciplinary team
  • demonstrate basic care to the primary caregiver
  • practice infection control measures and standard precautions
  • provide accurate documentation with visit itineraries
  • attend staff meetings
  • participate in at least one quality improvement activity yearly, as assigned by supervisor
  • complete documentation in a timely manner as outlined in orientation
