The Home Health Physical Therapist Assistant (PTA) role at CenterWell Home Health is critical in delivering rehabilitative care that enhances patients’ strength, mobility, and overall function. Reporting directly to the Clinical Manager, a PTA works hands-on with patients in their homes to administer physical therapy treatments as prescribed by licensed Physical Therapists. This position focuses on implementing therapeutic exercises and modalities such as massage, heat, cold, light, electrical stimulation, and traction to promote recovery from illness or injury.
The role requires meticulous documentation of patient progress to ensure timely clinical communication and quality care standards. PTAs also participate in educational meetings, departmental in-services, and maintain strict adherence to infection control and universal precautions. Effective communication with Physical Therapists and Branch Directors regarding any patient or equipment concerns is essential.

Job Requirements
- Associate degree in Physical Therapy from an American Physical Therapy Association
- Current state license to practice Physical Therapy
- Valid driver's license auto insurance and reliable transportation
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Comprehensive knowledge of the physical emotional social and biological changes that occur in the geriatric population
- Read write and speak fluent English

Job Duties
- Administer the physical therapy treatment plan to patients working onsite in their homes as developed by the Physical Therapist
- Administer non-complex active and passive manual therapeutic exercises therapeutic massage traction heat light cold water and electrical modalities to patients
- Document client records and submit clinical documentation to the office on time each week
- Attend educational meetings and coordinate department in-services
- Notify Physical Therapist or Branch Director of any equipment failures lack of supplies or staff or client changes
- Apply concepts of infection control and universal precautions in performing patient care activities
