Job Requirements

  • Must meet requirements of 484.115 condition of participation: personnel qualifications
  • Graduate of an accredited practical nurse or vocational nursing program
  • Three years of nursing experience preferred
  • Currently licensed LPN or LVN in Washington
  • Possess and maintain current CPR/BLS certification from AHA

Job Qualifications

  • Graduate of an accredited practical nurse or vocational nursing program
  • Currently licensed as LPN or LVN in Washington state
  • Possess and maintain current CPR/BLS certification from American Heart Association
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • Experience in home health or hospice nursing preferred

Job Duties

  • Provide high-quality nursing care to patients in their homes
  • Collaborate with interdisciplinary healthcare teams to develop and implement care plans
  • Monitor patients' health status and report changes to appropriate medical personnel
  • Maintain patient records and ensure compliance with healthcare regulations
  • Educate patients and families about care procedures and health maintenance
  • Adhere to ethical standards and agency core values in all aspects of nursing practice
  • Participate in ongoing training, professional development, and quality improvement initiatives
