Job Requirements
- Must meet requirements of 484.115 condition of participation: personnel qualifications
- Graduate of an accredited practical nurse or vocational nursing program
- Three years of nursing experience preferred
- Currently licensed LPN or LVN in Washington
- Possess and maintain current CPR/BLS certification from AHA
Job Qualifications
- Graduate of an accredited practical nurse or vocational nursing program
- Currently licensed as LPN or LVN in Washington state
- Possess and maintain current CPR/BLS certification from American Heart Association
- Strong communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Experience in home health or hospice nursing preferred
Job Duties
- Provide high-quality nursing care to patients in their homes
- Collaborate with interdisciplinary healthcare teams to develop and implement care plans
- Monitor patients' health status and report changes to appropriate medical personnel
- Maintain patient records and ensure compliance with healthcare regulations
- Educate patients and families about care procedures and health maintenance
- Adhere to ethical standards and agency core values in all aspects of nursing practice
- Participate in ongoing training, professional development, and quality improvement initiatives
