As a Home Health LPN (Licensed Practical Nurse) at CenterWell Home Health, you will play an essential role in delivering skilled nursing care directly to patients in their homes. Reporting to the Clinical Manager, this position requires hands-on engagement with patients, including those with complex medical needs, to provide individualized care plans developed in collaboration with patients, their families, and the broader healthcare team. The responsibilities include educating patients on disease management, treatment options, and self-care practices to empower them toward greater health autonomy. You will maintain meticulous records of patient visits and interventions to ensure continuity and quality of care. Your communication skills will be critical as you maintain consistent contact with patients, physicians, clinical managers, and other health professionals concerning patient conditions and changes. Additionally, you will actively participate in care coordination and discharge planning activities to support seamless patient transitions and improved outcomes.
This full-time role offers a competitive salary ranging from $53,000 to $73,000 annually, with additional pay based on visits or units ($33.00 to $47.00 per visit/unit). The Home Health LPN position provides a stable work schedule of 40 hours per week and is considered a patient-facing role, subject to mandatory tuberculosis screening and adherence to Humana’s driver safety program, which requires a valid driver’s license, auto insurance, and reliable transportation.

Job Requirements
- Current nursing license in the practicing state
- Valid drivers license auto insurance and reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting

Job Duties
- Provide skilled nursing care to patients working onsite in their homes under the direction of an RN
- Implement individualized care plans in collaboration with the patient family and healthcare team
- Educate patients and their families on disease management treatment options and self-care techniques
- Maintain accurate records of patient care including documentation of patient visit interventions and outcomes
- Maintain contact with patients physicians clinical manager(s) and other members of the healthcare team promptly regarding patient or family needs and status changes
- Participate in care coordination activities and discharge planning
