The Home Health Licensed Practical Nurse (LPN) position at CenterWell Home Health is a vital role focused on delivering skilled nursing care within patient homes, operating under the guidance of registered nurses and clinical managers. This role emphasizes one-on-one, personalized care tailored to each patient’s specific needs and recovery goals. As an LPN, you will implement individualized care plans, actively participate in care coordination and discharge planning, and provide education to patients and their families on disease management and self-care techniques. Accurate documentation of patient visits, interventions, and outcomes is integral to this position, ensuring continuity and quality of care.

This employment opportunity offers a competitive pay range from $53,000 to $73,000 annually for full-time positions, with a pay per visit/unit ranging from $33 to $47. Additional benefits include paid time off, a 401(k) retirement savings plan, employee assistance programs, and business travel insurance, among others. The role requires a valid nursing license in the practicing state, a current CPR certification, a valid driver’s license with reliable transportation, and at least two years of clinical experience as an LPN or LVN. Candidates with home health or hospice nursing experience are preferred.

Job Duties

  • Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN
  • implement individualized care plans in collaboration with the patient, family, and healthcare team
  • educate patients and their families on disease management, treatment options, and self-care techniques
  • maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes
  • maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes
  • participate in care coordination activities and discharge planning
