The Home Health Licensed Practical Nurse (LPN) position at CenterWell Home Health is a vital role focused on delivering skilled nursing care within patient homes, operating under the guidance of registered nurses and clinical managers. This role emphasizes one-on-one, personalized care tailored to each patient’s specific needs and recovery goals. As an LPN, you will implement individualized care plans, actively participate in care coordination and discharge planning, and provide education to patients and their families on disease management and self-care techniques. Accurate documentation of patient visits, interventions, and outcomes is integral to this position, ensuring continuity and quality of care.

This employment opportunity offers a competitive pay range from $53,000 to $73,000 annually for full-time positions, with a pay per visit/unit ranging from $33 to $47. The role requires a valid nursing license in the practicing state, a current CPR certification, a valid driver’s license with reliable transportation, and at least two years of clinical experience as an LPN or LVN.
