As a Home Health Licensed Practical Nurse (LPN) at CenterWell Home Health, you become a vital part of this caring community, dedicated to putting health first. Reporting directly to the Clinical Manager, the LPN role is centered on delivering skilled nursing care to patients within the comfort and privacy of their own homes. You will be entrusted with providing personalized, one-on-one care that supports patients in regaining independence and improving quality of life. Collaboration is key in this position—you will work alongside a dedicated team of physicians, registered nurses, and other clinicians to implement individualized care plans tailored to patient needs.

The position offers a full-time schedule of 40 hours per week, with competitive pay ranging from $53,000 to $73,000 per year, reflecting a good faith estimate of base salary adjusted by geographic location, skills, experience, and certifications. Additionally, you will receive pay per visit/unit ranging from $33.00 to $47.00. CenterWell emphasizes the importance of safety and compliance; as such, the role requires participation in Humana's Tuberculosis screening program and driver safety program, including maintaining valid licensure and insurance.

Your responsibilities will include educating patients and their families on disease management techniques, treatment options, and self-care strategies; maintaining accurate and timely patient records documenting care and outcomes; and maintaining vital communication among patients, physicians, clinical managers, and the wider healthcare team. Participating in care coordination and discharge planning ensures continuity of care and supports positive health outcomes.

Job Requirements

  • Current nursing license in the practicing state
  • Valid drivers license auto insurance and reliable transportation
  • Current CPR certification
  • Two years experience as an LPN or LVN in a clinical setting

Job Duties

  • Provide skilled nursing care to patients working onsite in their homes under the direction of an RN
  • Implement individualized care plans in collaboration with the patient family and healthcare team
  • Educate patients and their families on disease management treatment options and self-care techniques
  • Maintain accurate records of patient care including documentation of patient visit interventions and outcomes
  • Maintain contact with patients physicians clinical managers and other members of the healthcare team promptly regarding patient or family needs and status changes
  • Participate in care coordination activities and discharge planning
