Job Requirements

  • High school diploma or equivalent
  • 3-5 years of healthcare, customer service, patient relations or concierge experience
  • proficiency in Microsoft Office applications

Job Qualifications

  • High school diploma or equivalent
  • associate degree preferred
  • minimum 3-5 years of healthcare, customer service, patient relations, or hospitality concierge experience
  • strong communication and interpersonal skills
  • proficiency in Microsoft Office including Outlook, Word, and Excel
  • experience with electronic health records such as Epic preferred
  • organizational skills with attention to detail
  • ability to multitask and prioritize
  • self-starter with ability to work independently
  • ability to engage members telephonically and improve healthcare outcomes
  • knowledge of health insurance, local providers, and terminology
  • experience working with diverse cultural populations
  • problem-solving skills
  • ability to handle confidential information
  • experience in care coordination or doctor office environment preferred
  • certification or licensure such as Medical Assistant preferred
  • bilingual Spanish preferred

Job Duties

  • Coordinates patient flow throughout all scheduled appointments
  • interacts with customers, patients, members, and coworkers telephonically and through technology-based interactions including video, email, and text
  • reaches out to language line interpreter services when needed
  • schedules, tracks, and reconciles patient referrals and diagnostic tests
  • schedules tests per written workflows within Scheduler app via telephone or electronically and documents accordingly
  • demonstrates proficiency managing email in Outlook and using remote communication software such as Skype, Microsoft Teams, and WebEx
  • adheres to care management, privacy, confidentiality, and quality management processes in compliance with regulations and company policies
  • attends weekly team huddles to stay connected with Retail Health field and operations
  • provides superior customer service to internal and external clients
  • communicates with clinical team, patients, and families on recommended patient care plans
  • assists with completing and tracking patient forms and letters
  • notifies clinical care team of changes or barriers as appropriate
  • cross trains to other areas of retail health management and gaps in care
  • conducts orientation and training of staff as assigned
  • maintains respect and regard for the dignity of all patients, families, visitors, and employees
  • performs other responsibilities as assigned
