Job Requirements
- Academic degree in nursing
- Minimum of two years nursing experience
- Licensed, certified, or registered to practice nursing as required by law
- American Heart Association Basic Life Support (AHA BLS) certification
- Ability to collaborate with multidisciplinary healthcare teams
- Strong communication and patient advocacy skills
- Commitment to compassionate patient care
Job Qualifications

- Bachelor’s or master’s degree preferred
- Two years nursing experience
- Currently licensed, certified, or registered to practice nursing as required by state law

Job Duties
- Coordinate the care needs of assigned patients
- Develop comprehensive care plans across patient care continuum
- Partner with patients and primary physicians to customize care plans
- Collaborate with physicians, nurses, allied health professionals, and social workers
- Ensure timely delivery of appropriate tests and treatments
- Advocate for patients while balancing care needs and financial considerations
- Promote efficient and effective treatment outcomes
