Job Requirements

  • high school diploma or equivalent
  • six months of experience in human services preferred
  • must be 18 years of age
  • valid driver’s license in good standing
  • car registration and vehicle insurance if providing transportation
  • completion of all state-required training and certification in mandated timeframes
  • ability to handle physical work including exerting up to 100 pounds occasionally
  • travel required as needed

Job Duties

  • Training: attends orientation and ongoing training as directed
  • employee scheduling: works scheduled hours, arrives for work on time, stays on shift until replacement arrives, notifies supervisor as needed

  • workplace safety: complies with safety policies and reports work-related injuries
  • provides direct support or assistance according to individual plans
  • assists with socialization and behavioral development, personal care, housekeeping, recreational activities, transportation, community orientation, shopping, financial management, citizenship and other daily living activities
  • accurately documents progress and activities, reviews records to stay informed, maintains confidentiality, completes billing documentation as applicable
  • assists with money management and assures safety of individual funds and property
  • maintains healthy and professional relationships with individuals, families, guardians and case managers
  • implements customer service standards
  • reports any alleged abuse or neglect and other incidents according to policy
  • maintains confidentiality and respects rights of individuals served
  • accompanies individuals to medical appointments and relays information
  • accurately administers medications if assigned, reports errors and maintains medication security
  • monitors health, documents concerns, communicates with nurse or supervisor, calls 911 in emergencies
  • correctly uses medical equipment and reports malfunction
  • prepares or supports food preparation according to dietary plans and monitors mealtime
  • transports individuals safely and reports accidents and safety concerns
  • performs housekeeping duties to maintain a clean and safe environment
  • conducts minor repairs and maintenance tasks
  • participates in safety drills and protects individuals during emergencies
  • performs other related duties as required
