Job Requirements

  • High school diploma or equivalent
  • CNA license preferred
  • At least one year experience in long term care preferably in customer services
  • Ability to walk, reach, bend, lift, sit for extended periods, grasp, coordinate fine hand movements, push and pull
  • Ability to distinguish smells and temperatures with or without aid of mechanical devices

Job Qualifications

  • Ability to relate positively, effectively and appropriately and independently with patients and families
  • Experience working on Care Transition Unit preferred
  • Strong verbal and written communication skills in English
  • Sufficient computer skills to perform duties

Job Duties

  • Securing new customers through the facility tour process
  • Responsible for creating resident medical records in PointClickCare
  • Oversees daily census activities including clearing the dashboard in PointClickCare
  • Maintains the facility paper census and completes the daily census worksheets for the home office
  • Manages bed census and communicates room readiness, room changes and admission notifications to the admission assistant and IDT teams
  • Manages all swim lanes within the RESIDE software including reviewing and editing demographic and payer information to ensure contract accuracy
  • Works with residents or responsible parties to complete resident agreements via RESIDE within 72 hours of admission
  • Performs all other duties as assigned
