The current role available is for a Concierge specializing in admissions and resident transitions within the facility. This position is critical as it serves as the first point of contact for new admissions and their families, ensuring a smooth, welcoming, and well-organized integration into the community. The Concierge will be responsible for preparing rooms with durable medical equipment (DME), managing welcome packets containing essential information such as activity calendars, monthly newsletters, and facility policies, and assisting with practical needs like labeling clothes and facilitating laundry communications. The role also involves coordinating dietary needs with kitchen staff to prevent tray delays, reminding families about service and discharge planning meetings, and helping residents during transitions to long-term care settings within the facility.
Additional responsibilities include assisting residents with personal items like dentures, hearing aids, and eyeglasses, supporting discharge preparation by organizing residents' belongings, and aiding in grievance resolution to enhance resident satisfaction. The Concierge will facilitate administrative tasks like inputting referrals into the facility’s management software (PCC/CRM), signing in new admits, providing facility tours, and scanning admission documents. This position often requires escorting residents to appointments, ensuring their safety and comfort outside the facility.

Job Duties
- Meet and greet each new admission and their family
- ensure that the room is ready for new admissions to include DME, welcome packet and gift
- upkeep of the welcome packet including activity calendar, monthly newsletter, wifi code, resident rights, smoking policy, list of facility extensions and contact information
- assist with labeling of clothes and notify staff if family will do laundry
- ensure the diet communication slips get to the kitchen and the resident receives a tray without delay
- remind families of their delivery of service meetings and discharge planning meeting
- show resident the room, assist with packing and unpacking during transition to long term care
- assist residents with dentures, hearing aids or eyeglasses
- prepare resident for discharge by packing clothing and gathering laundry
- assist with grievance resolution
- ensure new admits receive coupon for free haircut or wash and set and accompany them to beauty shop
- assist admissions with input of referrals into PCC/CRM
- assist with signing in of new admits and giving tours
- assist with scanning admission packet into PCC
- escort residents on appointments
