Job Requirements
- High school diploma or GED
- current BLS and ACLS certifications
- required licensure as a paramedic within the state of employment
- ability to obtain Community Paramedicine certification or complete NEMSA Essentials Course or obtain CP-C licensure within two years
- effective verbal, written, and interpersonal communication skills
- demonstrated coaching and leadership abilities
- teaching and educational facilitation skills
- adherence to educational quality standards
- familiarity with computer applications
- team-oriented work ethic
- valid driver’s license and acceptable driving record
Job Qualifications
- High school diploma or GED

- ability to obtain Community Paramedicine (IBSC CP-C) certification or complete NEMSA Essentials Course or obtain CP-C licensure within two years of hire
- effective verbal, written, and interpersonal communication skills
- demonstrated ability to provide coaching and leadership
- teaching and educational facilitation skills
- familiarity with basic computer applications
- team-oriented
- valid driver’s license and acceptable driving record
- degree from an accredited college or university preferred
- previous trainer or instructor experience preferred
- previous critical care experience preferred
Job Duties
- Provide high-acuity care in the patient’s home setting for conditions such as infections, heart failure, pneumonia, and post-surgical recovery
- conduct thorough physical exams and assess the home environment for safety and factors impacting recovery
- maintain real-time communication with the virtual care team to report findings and collaborate on care decisions
- respond to crisis situations and rapid response alerts in the home environment
- deliver personalized education to patients and caregivers regarding disease management and medication regimens
- document all assessments, interventions, communications, and outcomes in the electronic health record system
- collaborate with interdisciplinary healthcare teams to ensure continuity of care
- participate in research initiatives, program development, and mentorship as assigned by leadership
