Job Description

This full-time leadership position at Tufts Medicine's High Pointe House Facility in Haverhill, MA, and skilled nursing facilities in Lowell, MA, involves overseeing one of the TM CAH Hospice teams. The role demands a strong clinical leader who will guide hospice and palliative care staff in applying best practices and clinical standards to optimize patient care. The position ensures collaboration with interdisciplinary teams that may include physicians, referral sources, hospital personnel, and other healthcare providers to identify and transition eligible patients to hospice care. It also includes management responsibilities for the inpatient hospice house alongside day-to-day clinical operations and compliance with regulatory and agency standards.

The ideal candidate will have a bachelor's degree in Nursing or an associate degree with relevant experience, a valid Massachusetts RN license, and at least five years of experience in palliative, hospice, or complex care. Additionally, progressive management experience of at least two years is required to effectively lead and supervise clinical and ancillary staff, ensuring that all staff deliver patient-centered, high-quality care. This leader will develop clinical protocols, promote adherence to policies and procedures, provide staff training and evaluation, and maintain robust communication among team members and external partners.

Job Requirements

Job Qualifications

Job Duties

  • Monitors and evaluates palliative care and hospice patient population census, trends, processes and outcomes at a facility base
  • Oversees day-to-day clinical operations at High Pointe House and patients in skilled nursing/assisted living facilities
  • Develops, implements and monitors clinical protocols and ensures adherence to palliative care and hospice standards
  • Promotes and maintains full implementation of palliative care and hospice programs and policies
  • Communicates with medical directors, hospital personnel, vendors, managers and staff to ensure optimal care
  • Trains staff and promotes awareness of palliative care and hospice programs
  • Develops standards of practice with Director of Hospice and Palliative Care
  • Prepares reports and utilizes data for cost and outcome management
  • Works collaboratively to address clinician competency and clinical protocols
  • Develops effective referral source relationships
  • Reviews plans of care with clinicians and ensures physician updates and orders
  • Conducts random chart audits and ensures compliance with regulations
  • Manages palliative and hospice clinicians and ancillary staff, providing training, supervision, and performance evaluations
  • Maintains positive communication with healthcare team, patients, families, and referral sources
  • Facilitates interdisciplinary team meetings and case conferences
  • Develops systems for patient and physician satisfaction assessment
  • Ensures compliance with regulatory agencies and agency policies
  • Monitors resource utilization and cost control
  • Documents findings and implements improvements based on trend analysis
  • Ensures quality care delivery and appropriate documentation for compliance
  • Approves invoices related to hospice care
  • Arranges staff coverage as needed and provides detailed handover reports
