Job Requirements

  • Education and licensure based on program contract requirements
  • High school diploma or equivalent required
  • Minimum 2 years clinical experience
  • LPN, RN, LCSW, or related licensure may be required
  • Master’s degree in Social Work or Mental Health or active unrestricted Illinois nursing license
  • Minimum 5 years post-graduate mental health experience
  • Minimum 3 years experience conducting mental health assessments
  • Reliable high-speed internet with minimum 20 Mbps download speed
  • Private and secure workspace
  • Ability to travel up to 80%

Job Qualifications

  • High school degree or equivalent
  • Minimum 2 years of clinical experience
  • Knowledge of community support programs and long-term care assessment
  • Understanding of medical and behavioral health diagnoses and prescribed medications
  • Ability to collect data, define problems, establish facts, and draw valid conclusions
  • Preferred licensure may include LPN, RN, LCSW based on contract
  • Master’s degree in Social Work, Mental Health, or related field or active unrestricted nursing license in Illinois
  • Minimum 5 years post-graduate mental health work experience
  • Minimum 3 years conducting mental health assessments

Job Duties

  • Complete medical and behavioral health assessments within contract requirements
  • Conduct on-site, video call, and telephonic assessments to determine individuals' needs for services and supports, eligibility, and level of care
  • Perform duties in compliance with Person First standards, HIPAA guidelines, and confidentiality policies
  • Travel as required up to 80% throughout specified geographic areas
  • Support admissions for Specialized Mental Health Rehabilitation Facilities and Supported Living Programs
  • Participate in Follow Up Visits and Colbert Dementia Review assessments
  • Perform other related duties as assigned
