Job Requirements

  • Degree in healthcare with at least two years relevant experience in a senior living setting
  • Current state licensure in physical therapy
  • Proficiency in internet browsers such as Chrome, Edge, Firefox
  • Advanced skills in Microsoft Office programs including Outlook, Excel, and Word
  • Experience working with electronic medical records and medical billing systems
  • Physical ability to lift/move 50 lbs, maintain stationary position, move freely, operate equipment, and reach above or below average standing height
  • Effective communication skills including ability to talk and hear regularly
  • Ability to stand, sit, stoop, walk frequently, and use hands and fingers to handle or feel
  • Specific vision abilities including close, distance, peripheral vision, color vision, depth perception, and focus adjustment
  • Willingness to comply with HIPAA and PHI confidentiality guidelines
  • Ability to work flexible hours and adapt to changing responsibilities

Job Qualifications

  • Degree in healthcare with at least two years relevant experience in a senior living setting
  • Current state licensure in physical therapy

  • Solid understanding of wellness offerings including physical, occupational, and speech therapy for seniors
  • General knowledge of laws and restrictions related to Assisted Living, Independent Living, and Memory Care
  • Exceptional communication and interpersonal skills

Job Duties

  • Lead the on-site EmpowerMe team in delivering high-quality healthcare services that are person-directed and produce outcomes aligning with company goals and objectives
  • Oversee day-to-day operations of assigned clinic(s) to optimize resident participation and productivity
  • Recruit, train, mentor, motivate and supervise a high-performing team, fostering a culture of collaboration and accountability
  • Create schedules to ensure patient care is commensurate with treatment plans
  • Review and approve therapists’ time via the payroll closeout process
  • Provide input on community budget and deliver budget goals
  • Implement clinical programs focused on the needs of residents in senior living communities
  • Identify residents who may benefit from services through screenings and communication with care staff
  • Ensure screening of all new residents within one week of moving in
  • Lead resident consent and intake processes efficiently and positively
  • Treat patients and families with respect and address psychosocial, cultural, ethnic, and spiritual needs
  • Comply with HIPAA and PHI guidelines, protecting confidentiality
  • Act as primary liaison between company and host community, maintaining communication with executive and nursing leadership
  • Promote EmpowerMe service offerings and provide in-services to care staff
  • Maintain professional relationships with physicians, care staff, administrative and interdisciplinary teams
  • Participate in tours and help prospective residents and families understand EmpowerMe services
  • Perform other duties consistent with qualifications, standards, and ethical practices, adapting to supervisory direction
  • Maintain company standards and align with vision for service quality and value creation
  • Ensure tasks are conducted with respect for patient/resident rights and confidentiality
