Job Requirements

  • Bachelor’s degree or equivalent combination of training and experience
  • experience in social service, counseling, or rehabilitation settings
  • direct experience with persons with disabilities
  • excellent communication skills
  • empathy and professionalism
  • ability to work independently and as part of a team
  • maturity and sound judgment
  • ability to write reports and manage time effectively
  • knowledge of community resources
  • must be at least 21 years old
  • clean driving record for past 3 years
  • reliable transportation
  • ability to operate a company vehicle
  • flexible availability including some evenings and weekends
  • adherence to company policies and guidelines

Job Qualifications

  • Bachelor’s degree in vocational rehabilitation, psychology, sociology, counseling and guidance, or related field
  • equivalent combination of training and experience in social services, counseling, or rehabilitation settings

Job Duties

  • Provide direction, supervision, and case management for assigned consumers
  • complete initial rehabilitation plans and assessments
  • develop and implement individual program plans
  • document participant progress
  • teach work habits, interaction, and self-help skills
  • facilitate training in production methods and productivity
  • assist with medication administration and participant transportation
  • provide job coaching and ongoing support to employers and participants
  • educate employers on disability awareness and ADA
  • complete necessary documentation including incident reports, program plans, and assessments
  • communicate and coordinate with involved agencies and support personnel
  • maintain accurate records and statistical data
  • participate in team meetings and in-service training
  • research industry trends and maintain current knowledge of regulations and policies
