Job Requirements
- Bachelor’s degree or equivalent combination of training and experience
- experience in social service, counseling, or rehabilitation settings
- direct experience with persons with disabilities
- excellent communication skills
- empathy and professionalism
- ability to work independently and as part of a team
- maturity and sound judgment
- ability to write reports and manage time effectively
- knowledge of community resources
- must be at least 21 years old
- clean driving record for past 3 years
- reliable transportation
- ability to operate a company vehicle
- flexible availability including some evenings and weekends
- adherence to company policies and guidelines
Job Qualifications
- Bachelor’s degree in vocational rehabilitation, psychology, sociology, counseling and guidance, or related field
- equivalent combination of training and experience in social services, counseling, or rehabilitation settings
- experience providing direct services to persons with physical, emotional, or developmental disabilities
- excellent written and verbal communication skills
- ability to interact effectively with diverse customers
- empathy towards disabled persons and their families
- ability to work independently and cooperatively
- strong planning and organizational skills
- maturity, good judgment, responsibility, and initiative
- effective interpersonal and decision-making skills
- ability to write clear and accurate reports

Job Duties
- Provide direction, supervision, and case management for assigned consumers
- complete initial rehabilitation plans and assessments
- develop and implement individual program plans
- document participant progress
- teach work habits, interaction, and self-help skills
- facilitate training in production methods and productivity
- assist with medication administration and participant transportation
- provide job coaching and ongoing support to employers and participants
- educate employers on disability awareness and ADA
- complete necessary documentation including incident reports, program plans, and assessments
- communicate and coordinate with involved agencies and support personnel
- maintain accurate records and statistical data
- participate in team meetings and in-service training
- research industry trends and maintain current knowledge of regulations and policies
