Job Requirements
- Minimum of an 8th grade education
- must be at least 18 years of age
- experience providing personal care and housekeeping preferred
- reliable, insured transportation or ability to use transit system
- ability to read, write and speak effectively in English or provide interpreter
- AHCCCS Direct Care Worker Training or current AZBN CNA/LNA
- current CPR and First Aid certification
- must have DPS Level 1 Fingerprint Clearance or ability to obtain within 90 days
- must pass Criminal Background check
- must pass APS Registry Check at new hire and annually
- must sign notarized Criminal History Affidavit at hire and every 3 years
- must provide proof of TB clearance
- able to lift 50 lbs
- able to sit or stand for long periods
- maintain training and certifications
- maintain confidentiality and professional dress code
- conform to background/fingerprint requirements
- reliable attendance and punctuality
Job Qualifications
- Minimum of an 8th grade education, high school education or GED preferred
- good communication skills and ability to follow directions, policies and procedures
- experience providing personal care and housekeeping, prefer three months to one year of experience as a caregiver
- additional experience or training may be required for specific services
- reliable, insured transportation or ability to utilize transit system to cover scheduled hours
- must be at least 18 years of age
- bilingual is a plus
- requires the ability to read, write and speak effectively in English or provide interpreter for interview, physical, and training classes
- AHCCCS Direct Care Worker Training or current AZBN CNA/LNA
- current CPR and First Aid Certification
Job Duties
- Help individuals with bathing, dressing, grooming, and hygiene to promote comfort and independence
- assist with meal preparation, light housekeeping, and mobility assistance
- foster meaningful connections and social engagement through companionship
- provide temporary relief for family caregivers while ensuring continuity of care
- assist with routine house cleaning, laundry, ambulation and transfers as indicated on service plans
- provide general supervision and companionship in accordance with service plans
- coordinate shopping and accompany members on errands, doctor appointments, and assist with arranging transportation
- accurately document services provided and complete electronic visit verification
- immediately report any changes in member's condition or incidents to supervisors
- maintain confidentiality regarding all personal client information
- maintain training, certifications, and a professional appearance adhering to dress code
- fulfill assignments with reliability and punctuality using personal conduct guidelines
- conform to ongoing background, fingerprint, attestation, and affidavit requirements
- perform other duties as assigned
