Job Description
This position is with an established assisted living community dedicated to providing compassionate, high-quality care to elderly and disabled residents. The community offers a supportive environment that preserves the dignity, safety, and comfort of each individual. As a Care Partner, you will play a vital role within the Health Services team, working under the leadership of the Health Services Director to support residents with activities of daily living and provide personalized assistance. The role of Care Partner is integral to maintaining the well-being and happiness of the residents while supporting a safe and nurturing environment. This position involves daily care assistance including helping with grooming, dressing, toileting, and personal hygiene tasks. You will monitor residents’ health by taking vital signs and recording any changes or unusual occurrences in their condition. Additionally, you will coordinate doctor appointments, assist with meals and laundry services, and respond compassionately to residents’ needs and emergencies. Your presence and interaction will enhance residents’ quality of life by providing companionship, answering call lights promptly, and facilitating recreational activities that promote physical and social engagement.
This care partner position requires individuals who demonstrate a genuine passion for elder care and the ability to work independently while managing multiple priorities effectively. You must be patient, empathetic, and physically capable of assisting residents who may need support with mobility or other challenging tasks. Holding a valid driver’s license, and certifications such as CPR and First Aid according to state requirements, are essential prerequisites. Experience or certification as a nursing assistant is desired, though high school diploma or equivalent is preferred. The physical demands include the ability to lift and assist residents weighing over 100 pounds and performing moderate physical activities like stooping, bending, and reaching, to maintain excellent care standards.

Job Requirements
- High school diploma or equivalent preferred
- Certification in nursing assistance desired
- CPR and First Aid certification as required per state guidelines
- Must maintain a current driver's license
- Ability to lift residents over 100 pounds with assistance
- Ability to stoop, bend, and reach
- Capability to handle moderate physical activity including lifting objects up to 50 pounds occasionally and 20 pounds frequently
Job Qualifications

Job Duties
- Takes vital signs and weighs residents
- Documents and charts on residents' changes and unusual occurrences
- Schedules doctor appointments as needed
- Assists residents with showers, grooming, dressing, and toileting
- Helps with activities of daily living as per individual care plans
- Assists with basic cleaning and maintenance tasks
- Cleans up after incidents while preserving resident dignity
- Provides assistance to residents who have fallen with safety and compassion
- Escorts residents to and from dining areas
- Manages laundry tasks including washing, drying, folding, and returning
- Assists in serving meals and taking orders
- Responds to call lights patiently and compassionately
- Communicates resident concerns to appropriate personnel
- Makes rounds to ensure resident safety and assistance
- Contacts health services director, doctors, and families regarding resident condition changes
- Attends required meetings and training sessions
- Maintains confidentiality and supports resident rights
- Provides tours to potential residents and families
- Ensures excellent customer service and communication
- Coordinates meal service coverage and dining room setup
- Performs other duties as assigned
