Job Requirements

High school diploma or GED equivalent
bachelor’s degree in healthcare, human services, or related field
willingness and ability to obtain CADC certification within two years
ability to work independently
effective interpersonal skills with individuals, agencies, and stakeholders
data management and reporting skills to ensure program objectives
understanding of substance abuse disorders
good communication skills
ability to work in diverse cultural settings

Job Qualifications

High school diploma or GED equivalent
bachelor’s degree in healthcare, human services, or related field from an accredited college or university
ability to obtain CADC certification within first two years
ability to function independently and work effectively with individuals and agencies
ability to manage data and reports to meet program goals
knowledge of human behavior and performance related to substance abuse treatment
strong communication and organizational skills
ability to work with diverse populations

Job Duties

Help lead initiatives aimed at improving healthcare outcomes and reducing health disparities for justice impacted populations
focus on increasing access to preventive, primary, and specialty care within the community
address health-related social needs impacting access and health outcomes including chronic conditions
enhance care integration, efficiency, and coordination across provider types and care levels
collaborate with healthcare and community partners to drive meaningful change
provide clients with comprehensive medical care and medication-assisted recovery as needed
refer and link clients to community-based social services addressing social determinants of health
respect and provide culturally appropriate, individualized treatment to diverse populations
engage and educate clients on treatment participation and social supports
maintain relationships with community programs and attend linkage appointments
conduct outreach and follow-up with clients, families, and treatment providers
