Job Requirements
- High school diploma or GED equivalent
- bachelor’s degree in healthcare, human services, or related field
- willingness and ability to obtain CADC certification within two years
- ability to work independently
- effective interpersonal skills with individuals, agencies, and stakeholders
- data management and reporting skills to ensure program objectives
- understanding of substance abuse disorders
- good communication skills
- ability to work in diverse cultural settings
Job Qualifications
- High school diploma or GED equivalent
- bachelor’s degree in healthcare, human services, or related field from an accredited college or university
- ability to obtain CADC certification within first two years
- ability to function independently and work effectively with individuals and agencies
- ability to manage data and reports to meet program goals
- knowledge of human behavior and performance related to substance abuse treatment
- strong communication and organizational skills
- ability to work with diverse populations
Job Duties
- Help lead initiatives aimed at improving healthcare outcomes and reducing health disparities for justice impacted populations
- focus on increasing access to preventive, primary, and specialty care within the community
- address health-related social needs impacting access and health outcomes including chronic conditions
- enhance care integration, efficiency, and coordination across provider types and care levels
- collaborate with healthcare and community partners to drive meaningful change
- provide clients with comprehensive medical care and medication-assisted recovery as needed
- refer and link clients to community-based social services addressing social determinants of health
- respect and provide culturally appropriate, individualized treatment to diverse populations
- engage and educate clients on treatment participation and social supports
- maintain relationships with community programs and attend linkage appointments
- conduct outreach and follow-up with clients, families, and treatment providers
