The role of Care Concierge is crucial within Curana’s interdisciplinary care team. As an on-site healthcare liaison, the Care Concierge acts as a vital bridge between senior living facility residents, healthcare providers, and Curana’s virtual support network. This position requires a passionate and detail-oriented professional who understands the nuances of senior healthcare, chronic care management, and effective communication across multidisciplinary teams.
Primary responsibilities of the Care Concierge include conducting monthly rounds for patients enrolled in chronic care management (CCM) or dementia programs, documenting changes in resident health status such as hospitalizations or discharges, enrolling new residents into CCM services, and facilitating patient consent processes. Additionally, the Care Concierge manages electronic health records to maintain accurate patient data, coordinates with facility staff and providers for regulatory visits, orders, and prior authorizations, and supports telehealth consultations and appointment scheduling. These duties are pivotal in ensuring seamless healthcare delivery and improving patient outcomes.
Beyond clinical support, the Care Concierge must exhibit strong organizational and communication skills, allowing them to effectively collaborate with Curana’s interdisciplinary care team and maintain continuous, responsive interaction with patients and caregivers through various communication channels like phone and patient portals. Job Requirements
- High school diploma or equivalent
- Registered or certified medical assistant certification required
- Minimum of 1 year experience in a medical office, community engagement, or related field
- Ability to interpret clinical documentation including medical records and lab results
- Willingness and ability to travel to multiple local senior living communities 3-4 days per week
- Proficient with computer applications including Microsoft Office
- Excellent communication and interpersonal skills
- Valid driver’s license, auto insurance, and acceptable driving record may be required depending on travel needs
Job Qualifications
- Registered or certified medical assistant
- One or more years of experience working in a medical office, community engagement, or related fields
- Ability to interpret medical records, lab results, and appointment notes
- Experience in collecting clinical information and surfacing changes to the care team
- Willingness to travel to local senior living communities
- Proficient computer skills and knowledge of Microsoft Office applications
- Excellent organizational and time management skills
- Experience working with chronic care management or care management programs preferred
Job Duties
- Conduct monthly in-person rounds for patients enrolled in chronic care management or dementia programs
- Identify and document resident status changes such as deceased residents, hospitalizations, rehab placements, and discharge home
- Identify and enroll new residents into CCM services
- Introduce CCM programs to residents and caregivers and obtain consent
- Conduct comprehensive chart review for each resident
- Complete the CCM pre-visit template based on clinical findings
- Address patient and durable power of attorney inquiries via telephone
- Respond to patient or caregiver messages via the patient portal
- Assist patients with scheduling follow-up appointments with Curana providers or specialists
- Manage electronic health records to ensure accurate patient records
- Coordinate regulatory skilled nursing facility visits with facility staff and providers
- Coordinate documents needed for review or signature by a provider
- Facilitate provider orders and escalate findings
- Support provider scheduling
- Assist with prior authorizations
- Obtain patient records and diagnostic test results
- Organize telehealth consultations
- Collaborate with the interdisciplinary care team to ensure seamless communication
- Perform other duties as assigned
