The Care Concierge role is an integral part of Curana's Interdisciplinary Care Team. The Care Concierge works on-site at Senior Living Facilities, serving as the essential link between patients, healthcare providers, and Curana's virtual support team. This position is pivotal in ensuring residents receive thorough chronic care management and specialized dementia care through programs like Chronic Care Management (CCM) and Guiding an Improved Dementia Experience (GUIDE). The role encompasses patient support, provider support, and communication facilitation within the care team network.
The Care Concierge is responsible for conducting monthly in-person rounds with enrolled patients to track their care needs meticulously. Their duties include identifying and documenting changes in resident status such as hospitalizations or discharges and enrolling new residents into CCM services by introducing these programs and obtaining consent. They perform comprehensive chart reviews and maintain detailed records through electronic health records (EHR), ensuring that patient information is accurate and current.
Working closely with healthcare providers, Care Concierges manage provider scheduling, coordinate regulatory visits and necessary documentation, assist with prior authorizations, and facilitate telehealth consultations. They act as a critical communication bridge, supporting both patients and providers while enhancing collaboration among the entire Interdisciplinary Care Team.
This role requires a detail-oriented individual skilled in interpreting medical records, managing clinical information, organizing appointments, and providing exceptional support. Candidates must also be prepared for travel to various local Senior Living Communities three to four days a week to fulfill their responsibilities effectively.

Job Requirements
- Registered or certified medical assistant
- At least one year of relevant experience in medical office or community engagement
- Ability to interpret medical records and lab results
- Proficient in Microsoft Office
- Strong organizational and time management skills
- Willingness to travel to assigned senior living communities three to four days per week
- Valid driver’s license, current auto insurance, and acceptable driving record may be required with notice
- Ability to communicate effectively and collaborate within a care team
Job Qualifications
- Registered or certified medical assistant
- Minimum one year experience in medical office, community engagement, or related field
- Ability to interpret medical records, lab results, and appointment notes
- Experience collecting clinical information and communicating changes to care team
- Proficient computer skills including Microsoft Office applications
- Excellent organizational and time management abilities
- Willingness to travel to local senior living communities
- Experience with chronic care management programs preferred
Job Duties
- Conduct monthly in-person rounds for patients enrolled in chronic care management and dementia care programs
- Work on-site to identify and document resident status changes such as deceased residents, hospitalizations, rehab placements, and discharges
- Identify and enroll new residents into chronic care management services
- Introduce chronic care management programs to residents and caregivers and obtain consent
- Conduct comprehensive chart reviews and complete pre-visit templates based on clinical findings
- Address patient and durable power of attorney inquiries via telephone and patient portal
- Assist patients with scheduling follow-up appointments with providers or specialists
- Manage electronic health records to ensure accurate patient records
- Coordinate regulatory skilled nursing facility visits and communicate with facility staff and providers
- Coordinate documents needed for provider review or signatures
- Facilitate provider orders and escalate clinical findings
- Support provider scheduling and assist with prior authorizations
- Obtain patient records and diagnostic test results and organize telehealth consultations
- Collaborate with interdisciplinary care teams to ensure seamless communication
- Perform other duties as assigned
