At Home Harmony is an innovative home-based medical and pharmacy practice located in Richmond, VA, established in 2022. We specialize in delivering exceptional healthcare services targeted at aging adults, particularly those grappling with complex chronic conditions. Our Care Concierge Team is composed of highly skilled and experienced Registered Nurses (RNs), Licensed Practical Nurses (LPNs), and Medical Assistants (MAs), all driven by a deep passion for clinical support and case management in a home-based setting. The team plays an integral role in supporting our medical providers, patients, and their families or caregivers by ensuring smooth coordination of care and clinical management from a hybrid work environment that includes company headquarters, remote home offices, and senior living communities.

We are seeking an experienced Care Concierge / RN to join our dynamic team. This remote role involves providing clinical support and case management primarily over the phone, incorporating electronic medical record (EMR) management and coordination with various healthcare stakeholders. The Care Concierge / RN will be responsible for triaging patient concerns, facilitating scheduling, managing EMR inbox and documentation, and ensuring diagnostic testing is ordered and tracked appropriately. This position requires strong clinical judgment, excellent communication skills, and the ability to manage multiple priorities effectively in a remote work setting.

Additionally, the role includes case management duties such as developing personalized care plans, assisting patients during transitions of care like hospital discharges and rehabilitation admissions, and supporting home health orders and ongoing chronic care management. The successful candidate will occasionally conduct in-person assessments, manage remote patient monitoring devices, and collaborate closely with the At Home Harmony provider team and support staff to deliver comprehensive chronic care solutions.

Job Requirements

  • Registered Nurse license active and unrestricted in Virginia
  • current Basic Life Support certification
  • at least 5 years experience in relevant healthcare settings
  • minimum 2 years experience with electronic medical records
  • strong communication and technology skills
  • ability to work independently in a remote setting
  • valid driver’s license with a clean driving record
  • physical ability to lift and carry equipment up to 50 pounds
  • ability to walk up and down several flights of stairs
  • adaptable and motivated to meet changing work demands

Job Qualifications

  • Registered Nurse (RN) license active and unrestricted in Virginia
  • current Basic Life Support (BLS) certification
  • minimum 5 years experience in case management, disease management, home health care, hospital, or outpatient office setting
  • experience working with geriatrics, family practice, internal medicine, cardiac or pulmonary specialties
  • proficiency with electronic medical records (EMR) systems, Athena EMR preferred
  • strong knowledge of chronic care monitoring and management
  • strong technology skills and familiarity with multiple systems
  • excellent oral and written communication skills
  • professional, adaptable, and friendly demeanor
  • ability to self-direct and prioritize tasks in a remote work environment
  • familiarity with physician office operations preferred
  • valid driver’s license with a clean driving record

Job Duties

  • Providing clinical support remotely via phone
  • triaging patient concerns and facilitating scheduling in EMR
  • recording and managing patient data in EMR
  • managing EMR inbox and returning patient calls
  • ordering, tracking, and posting diagnostic test results
  • ordering durable medical equipment (DME)
  • monitoring remote patient devices and contacting patients for abnormal readings
  • creating personalized care plans for patients
  • assisting patients with care transitions including hospital and rehab discharges
  • scheduling follow-up appointments and managing home health orders
  • completing in-person assessments for remote patient monitoring device setup as needed
  • educating patients on disease management and risk reduction
  • documenting patient interactions and clinical information in EMR and other software
  • promoting coordinated medical and pharmacy care model
