We are currently seeking a dedicated Aftercare Concierge to join our Intensive Inpatient Program based in Ennis, TX. This is a full-time, exempt position offering an annual salary range of $45,000 to $55,000 with a schedule primarily Monday through Friday, including some weekends as needed. The Aftercare Concierge plays a crucial role in ensuring the continuity of care for clients as they transition from our treatment programs to outpatient services and community-based recovery support. This position involves close collaboration with clinical and case management teams to develop personalized discharge plans tailored to each client’s unique needs and goals.
In this role, you will act as a vital link between clients, their families, the clinical team, outpatient providers, and community resources. By coordinating referrals, confirming continuity of care, and facilitating communication among all parties involved, you help reduce risks of relapse and support sustained recovery. Your responsibilities include creating customized aftercare plans incorporating outpatient therapy, psychiatry, intensive outpatient and partial hospitalization programs, sober living, peer support, and alumni engagement initiatives.
You will manage detailed documentation and updates in electronic medical records (EMR), track post-discharge follow-up outcomes, and maintain comprehensive resource lists to assist clients effectively. Additionally, you will support smooth transitions by arranging appointments, transportation, and housing when needed, while advocating for client needs and preferences throughout the discharge planning process.
This role requires strong organizational and communication skills, empathy, and cultural competence to engage clients compassionately and effectively. Experience with behavioral health systems, recovery services, and familiarity with EMR systems are preferred qualifications. Job Requirements
- Associate or bachelor’s degree in psychology, social work, public health, or related field preferred
- 1-2 years of experience in discharge planning, case management, or recovery services
- Familiarity with behavioral health systems
- Strong organizational skills
- Effective communication skills

Job Qualifications

Job Duties
- Collaborate with clients with their clinical team, case management, and medical teams to understand client needs and discharge goals
- Develop customized aftercare plans including outpatient therapy, psychiatry, IOP/PHP referrals, sober living, peer support, and alumni engagement
- Coordinate referrals, obtain documentation, and confirm continuity of services prior to discharge
- Communicate aftercare plans with clients and families to ensure understanding and agreement
- Track and follow up on post-discharge referrals and outcomes
- Maintain up-to-date lists of community resources, treatment providers, and recovery organizations
- Support smooth transitions by helping schedule appointments and ensuring transportation or housing arrangements are confirmed
