Job Requirements
- Associate or bachelor’s degree in psychology, social work, public health, or related field preferred
- 1-2 years of experience in discharge planning, case management, or recovery services
- Familiarity with behavioral health systems
- Strong organizational skills
- Effective communication skills
- Ability to collaborate with multidisciplinary teams
- Compassionate and client-centered mindset
Job Qualifications
- Associate or bachelor’s degree in psychology, social work, public health, or related field preferred
- 1-2 years of experience in discharge planning, case management, or recovery services
- Familiarity with behavioral health systems, outpatient treatment models, and recovery resources
- Strong organizational, communication, and follow-through skills
- Compassionate, client-centered approach with cultural competence
- Experience using EMR systems and referral tracking tools preferred
Job Duties
- Collaborate with clients with their clinical team, case management, and medical teams to understand client needs and discharge goals
- Develop customized aftercare plans including outpatient therapy, psychiatry, IOP/PHP referrals, sober living, peer support, and alumni engagement
- Coordinate referrals, obtain documentation, and confirm continuity of services prior to discharge
- Communicate aftercare plans with clients and families to ensure understanding and agreement
- Track and follow up on post-discharge referrals and outcomes
- Maintain up-to-date lists of community resources, treatment providers, and recovery organizations
- Support smooth transitions by helping schedule appointments and ensuring transportation or housing arrangements are confirmed
